低分子肝素配合丹参川芎在孕早期子宫动脉阻力异常患者中的临床应用疗效观察

Clinical Effect of Low Molecular Weight Heparin Combined with Salvia Miltiorrhiza Chuanxiong on Abnormal Uterine Artery Resistance in Early Pregnancy

目的 探讨低分子肝素配合丹参川芎在孕早期子宫动脉阻力异常患者中的临床应用疗效。方法 选择2020年1月至2022年9月超声提示孕早期子宫动脉阻力异常患者70例为对象,随机数字表法分为两组各35例。两组均给予口服地屈孕酮治疗,对照组采用低分子肝素治疗,观察组联合丹参川芎治疗,7 d为1个疗程,两组完成1~3个疗程,并完成12周随访,比较两组有效率、激素水平、子宫动脉血流频谱、稽留流产及药物不良反应发生率。结果 观察组2个疗程治疗后有效率为94.28%,高于对照组的85.71%(P <0.05);两组治疗2个疗程后激素水平及子宫动脉血流频谱得到改善,观察组血P、β-HCG水平高于对照组,PI、S/D低于对照组(P <0.05);两组用药期间恶心呕吐、血压波动、肝肾异常及腹泻便秘发生率无统计差异(P> 0.05);观察组随访12周内稽留流产发生率低于对照组(P <0.05)。结论 低分子肝素联合丹参川芎应用于孕早期子宫动脉阻力异常患者,能获得较高的有效率,有助于改善患者激素及子宫动脉血流频谱,能降低稽留流产率,且药物安全性较高。

Objective To investigate the clinical effect of low molecular weight heparin combined with Danshen Chuanxiong in patients with abnormal uterine artery resistance in early pregnancy.Methods From January 2020 to September 2022,70 patients with abnormal uterine artery resistance in early pregnancy were selected and randomly divided into two groups with 35 cases in each group.Both groups were treated with oral dydrogesterone,the control group was treated with low molecular weight heparin,the observation group was treated with salvia miltiorrhiza chuanqi,7 days as a course of treatment,the two groups completed 1-3 courses of treatment,and completed 12 weeks of follow-up.The effective rate,hormone level,uterine artery blood flow spectrum,missed abortion and incidence of adverse drug reactions were compared between the two groups.Results After two courses of treatment,the effective rate of observation group was 94.28%,which was higher than that of control group 85.71%(P0.05).The incidence of missed abortion in the observation group was lower than that in the control group within 12 weeks of follow-up(P<0.05).Conclusion Low molecular weight heparin combined with salvia miltiorrhiza ligusticum in patients with abnormal uterine artery resistance in early pregnancy can achieve high effective rate,help to improve the hormone and uterine artery blood flow spectrum,reduce the missed abortion rate,and has high safety.
