冷冻切片与细胞学涂片在卵巢肿瘤病理诊断中的应用效果分析

Analysis of the effect of frozen sections and cytological smears in the pathological diagnosis of ovarian tumors

目的探讨与分析冷冻切片与细胞学涂片在卵巢肿瘤病理诊断中的应用效果。方法选取175例行卵巢肿瘤手术患者,术中对送检组织、肿瘤标本分别进行单纯细胞学涂片、单纯冷冻切片及结合检查进行卵巢肿瘤病理诊断。比较细胞学涂片、冷冻切片单独检查所用时间和二者单独和结合检查的敏感性、特异性、诊断符合率。结果冷冻切片检查所用时间(1735±5.42)min,细胞学涂片检查所用时间(5.87±1.10)min,细胞学涂片检查所用时间显著短于冷冻切片检查,差异有统计学意义(P<0.05)。冷冻切片诊断卵巢肿瘤的敏感性78.95%(75/95),特异性100%(80/80),诊断符合率88.57%(155/175)。细胞学涂片诊断卵巢肿瘤的敏感性89.47%(85/95),特异性为100%(80/80),诊断符合率94.29%(165/175)。冷冻切片联合细胞学涂片诊断卵巢肿瘤的敏感性95.24%(100/105),特异性100%(70/70),诊断符合率97.14%(170/175)。冷冻切片联合细胞学涂片诊断卵巢肿瘤的敏感性、诊断符合率均明显高于冷冻切片检查法、单纯细胞学涂片检查法,差异有统计学意义(χ^(2)=12.214,P=0.001)。结论在卵巢肿瘤病理诊断中使用冷冻切片与细胞学涂片可对其疾病诊断具有积极作用,保证疾病判断的正确性。

Objective To investigate and analyze the application effect of frozen section and cytological smear in the pathological diagnosis of ovarian tumors.Methods 175 patients with ovarian tumors underwent surgery were selected.Cytological smear,frozen section and combined examination were performed for pathological diagnosis of ovarian tumors.The sensitivity,specifi city and diagnostic coincidence rate of cytological smear and frozen section were compared.Results The time of cytological smear examination was(1735±5.42)min,and that of cytological smear examination was(5.87±1.10)min.The time of cytological smear examination was signifi cantly shorter than that of frozen smear examination,and the diff erence was statistically signifi cant(P<0.05).The sensitivity and specifi city of frozen section for the diagnosis of ovarian tumor were 78.95%(75/95),100%(80/80)and 88.57%(155/175).The sensitivity and specifi city of cytology smear in the diagnosis of ovarian tumor were 89.47%(85/95),100%(80/80),and 94.29%(165/175).The sensitivity and specificity of frozen section combined with cytology smear in the diagnosis of ovarian tumor were 95.24%(100/105),100%(70/70),and the diagnostic coincidence rate was 97.14%(170/175).The sensitivity and diagnosis coincidence rate of cryopreservation combined with cytology smear in the diagnosis of ovarian tumor were signifi cantly higher than that of cryopreservation combined with cytology smear,the diff erence was statistically signifi cant(χ^(2)=12.214,P=0.001).Conclusion frozen sections and cytological smears play an important and decisive role in the pathological diagnosis of ovarian tumors,and can eff ectively improve the accuracy of pathological diagnosis of ovarian tumors.
