CT与MRI在卵巢肿瘤诊断中的价值比较

Comparison of CT and MRI in the diagnosis of ovarian tumors

目的探讨CT与磁共振成像(MRI)在卵巢肿瘤诊断中的临床价值。方法选取47例卵巢肿瘤患者,采用CT与MRI进行检查,将组织病理学检查结果作为金标准,比较CT、MRI诊断卵巢肿瘤的结果与效能、恶性卵巢肿瘤分期准确率和影像学表现。结果CT诊断卵巢肿瘤真阳性20例,真阴性11例,假阳性7例,假阴性9例。MRI诊断卵巢肿瘤真阳性28例,真阴性17例,假阳性1例,假阴性1例。MRI诊断卵巢肿瘤的敏感性和特异性、准确率均高于CT,差异有统计学意义(P<0.05)。MRI诊断恶性卵巢肿瘤Ⅰ期、Ⅱ期的准确率均为100%,高于CT的0、60.00%,差异有统计学意义(P<0.05)。CT、MRI诊断恶性卵巢肿瘤Ⅲ期、Ⅳ期的准确率比较,差异无统计学意义(P>0.05)。结论卵巢肿瘤可采用CT和MRI诊断,而MRI的诊断价值优于CT,诊断敏感性和特异性、准确率均高,尤其对Ⅰ期和Ⅱ期的恶性卵巢肿瘤诊断准确率高,可通过患者的影像学表现分析判断患者的病情。

Objective To investigate the value of CT and magnetic resonance imaging(MRI)in the diagnosis of ovarian tumors.Methods 47 patients with ovarian tumors were selected and examined by CT and MRI.The results of histopathology were taken as the gold standard.The results and efficacy of CT and MRI in the diagnosis of ovarian tumors,the staging accuracy and imaging manifestations of malignant ovarian tumors were compared.Results CT diagnosis of ovarian tumors was true positive in 20 cases,true negative in 11 cases,false positive in 7 cases,and false negative in 9 cases.MRI diagnosis of ovarian tumors was true positive in 28 cases,true negative in 17 cases,false positive in 1 case,and false negative in 1 case.The sensitivity,specifi city and accuracy of MRI in the diagnosis of ovarian tumors were higher than those of CT,and the differences were statistically signifi cant(P<0.05).The accuracy of MRI in the diagnosis of stageⅠandⅡmalignant ovarian tumors was 100%,which was higher than that of CT(0,60.00%),and the difference was statistically signifi cant(P<0.05).There was no signifi cant diff erence in the accuracy of CT and MRI in the diagnosis of stageⅢandⅣmalignant ovarian tumors(P>0.05).Conclusion Ovarian tumors can be diagnosed by CT and MRI,and the diagnostic value of MRI is better than CT,with high diagnostic sensitivity,specifi city and accuracy,especially for stageⅠand stageⅡmalignant ovarian tumors.The patient's condition can be judged by the analysis of the patient's imaging performance.
