基于加速康复外科理念下妇科腹腔镜术后早期拔除尿管的临床研究

Clinical study of early removal of urinary tube after gynecological laparoscopy based on the concept of accelerated rehabilitation surgery

目的研究基于加速康复外科理念下妇科腹腔镜术后早期拔除尿管的临床效果。方法选取90例行妇科腹腔镜手术留置尿管患者,根据手术时间先后顺序分为对照组与观察组,各45例。对照组患者手术后第1天下午拔除尿管,观察组患者基于加速康复外科理念下于术后6~8 h拔除尿管。对比两组患者下床活动时间、肛门排气情况、尿路不适感率及拔除尿管意愿率。结果观察组患者下床活动时间及肛门排气时间均短于对照组,差异有统计学意义(P<0.05)。观察组尿路不适感率15.56%低于对照组的51.11%,拔除尿管意愿率20.00%低于对照组的57.78%,差异有统计学意义(P<0.05)。结论基于加速康复外科理念下妇科腹腔镜术后6~8 h拔除尿管可缩短患者下床活动时间及肛门排气时间,增加患者的舒适度,利于患者早期康复。

Objective To study the clinical effect of early removal of urinary tube after gynecological laparoscopy based on the concept of accelerated rehabilitation surgery.Methods 90 patients who underwent gynecological laparoscopic surgery with indentured urinary catheter were selected and divided into control group and observation group according to the operation time sequence,45 cases in each group.The urinary tube was removed in the afternoon of the first day after surgery in the control group,and the urinary tube was removed in the observation group 6~8 h after surgery based on the concept of accelerated recovery.The time of getting out of bed,the condition of anal exhaust,the rate of urinary discomfort and the willingness to remove urinary tube were compared between the two groups.Results The time of getting out of bed and the time of anal exhaust in observation group were shorter than those in control group,the difference was statistically signifi cant(P<0.05).The rate of urinary tract discomfort in the observation group was 15.56%lower than that in the control group(51.11%),and the willingness rate of catheter removal was 20.00%lower than that in the control group(57.78%),the difference was statistically significant(P<0.05).Conclusion Based on the concept of accelerated rehabilitation,removal of urinary tube 6~8 h after gynecological laparoscopy can shorten the time of getting out of bed and the time of anal exhaust,increase the comfort of patients,and facilitate early recovery of patients.
