阿帕替尼联合化疗药物治疗晚期食管癌效果及不良反应的Meta分析

Meta analysis of the efficacy and adverse reactions of apatinib combined with chemotherapy in the treatment of advanced esophageal cancer

目的进行阿帕替尼联合化疗药物治疗晚期食管癌效果及不良反应的Meta分析。方法计算机检索数据库,检索时间截止至2021年6月4日。采用Jadad量表、MINORS量表对纳入的随机、非随机临床对照试验进行质量评价并提取相关数据并交替核对。结局指标包括1、3个月的疾病客观有效率(ORR)及疾病控制率(DCR),1年生存率,不良反应(总不良反应、消化道反应、手足综合征、高血压、乏力、蛋白尿、白细胞减少、血小板减少、血红蛋白减少)的发生率。结果共纳入9篇研究文献,644例病例。Meta分析显示阿帕替尼联合化疗治疗晚期食管癌的1、3个月的客观缓解率、疾病控制率、1年生存率均高于单独化疗组(P<0.05),2组间不良反应比较差异无统计学意义(P>0.05)。结论阿帕替尼联合化疗治疗晚期食管癌效果良好,可提高近期疗效、疾病控制率、1年生存率,且安全性好。

Objective To conduct meta-analysis on the efficacy and adverse reactions of apatinib combined with chemotherapy in the treatment of advanced esophageal cancer.Methods The database will be searched by computer until June 4th,2021.Jadad scale and minors scale were used to evaluate the quality of randomized and non randomized clinical controlled trials,and relevant data were extracted and checked alternately.Outcome measures included objective disease response rate(ORR),disease control rate(DCR)at 1 and 3 months and 1-year survival rate.Adverse reactions include the incidence of total adverse reactions,gastrointestinal reactions,hand foot syndrome,hypertension,fatigue,proteinuria,leukopenia,thrombocytopenia,and hemoglobinopenia.Results A total of 9 research literatures involving 644 cases were included.Meta analysis showed that the objective remission rate,disease control rate of 1 and 3 months and 1-year survival rate of apatinib combined with chemotherapy for advanced esophageal cancer were higher than those of chemotherapy alone(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion Apatinib combined with chemotherapy is effective in the treatment of advanced esophageal cancer,which can improve the short-term efficacy,disease control rate and 1-year survival rate of patients.And the drug safety is good.
