摘要
目的:探索正电子放射性药物^(18)F-PSMA-1007在膀胱内核素滞留的影响因素,为优化检查流程,提高图像质量提供参考。方法:回顾性纳入因前列腺癌行^(18)F-PSMA PET/CT显像的患者47例,收集显像前后2周内患者肝、肾功指标,手工测量PET/CT图像上肝脏、双肾皮质、膀胱内核素分布的最大标准化摄取值(maximum standardized uptake value,SUV_(max))。使用Spearman相关分析膀胱内核素SUV_(max)与患者肝、肾功能指标及肝SUV_(max)、双肾SUV_(max)的相关性,具有统计学差异的指标进一步纳入线性回归分析模型,采用向前回归法获得影响膀胱内核素滞留的独立影响因素。结果:膀胱内核素分布SUV_(max)与肝脏SUV_(max)(r=-0.438,P=0.003)、总胆固醇(r=-0.441,P=0.004)呈显著负相关,与血糖(r=0.552,P<0.001)、尿素(r=0.391,P=0.010)存在显著正相关性。回归分析发现,血清中葡萄糖与尿素水平是膀胱内核素滞留程度的独立影响因素。结论:肾功中葡萄糖与尿素是影响^(18)F-PSMA-1007 PET/CT成像中膀胱出现核素滞留的重要因素,图像判读时应予以注意。
Objective:To explore the clinical influencing factors of positron radiopharmaceutical^(18)F-PSMA-1007 in nucleic retention in the bladder.Methods:47 patients who underwent^(18)F-PSMA PET/CT imaging for prostate cancer were retrospectively enrolled.Collect the liver and kidney function indexes of patients within 2 weeks before and after imaging.Manually measure the SUV_(max) of liver,kidney cortex and urine in the bladder on PET/CT imaging.Spearman was used to analyze the correlation between the SUV_(max) of urine in the bladder and the liver function indexes,kidney function indexes,liver SUV_(max) and double kidney SUV_(max).The statistically different indexes were further included in the linear regression analysis model,and the forward regression method was used to obtain the inner nucleic affecting the bladder.Results:Bladder urine SUV_(max) was significantly negatively correlated with liver SUV_(max)(r=-0.438,P=0.003),total cholesterol(r=-0.441,P=0.004),and was significantly positively correlated with glucose(r=0.552,P<0.001),urea(r=0.391,P=0.010).Regression analysis found that glucose and urea were independent factors influencing the retention of nucleic in the bladder.Conclusion:Glucose and urea are important factors that affect the occurrence of nuclide retention in the bladder in^(18)F-PSMA-1007 PET/CT imaging.
