地特胰岛素联合门冬胰岛素治疗妊娠期糖尿病对母婴结局的影响

Effect of insulin detemir combined with insulin aspart on maternal and infant outcomes in gestational diabetes mellitus

目的分析妊娠期糖尿病患者采用地特胰岛素联合门冬胰岛素治疗的效果及对母婴结局的影响。方法选取本院收治的妊娠期糖尿病患者32例为研究对象,采用电脑软件随机取数完成分组,将取得奇数号者纳入对照组(16例,单一门冬胰岛素治疗),将取得偶数号者纳入研究组(16例,门冬胰岛素联合地特胰岛素治疗),比较两组患者治疗前后的血糖指标,并记录妊娠并发症发生情况。结果治疗前,两组空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbAlc)水平比较,差异无统计学意义(P>0.05);治疗后,两组FPG、2 h PG、HbAlc水平均下降,且研究组各指标水平均低于对照组,差异有统计学意义(P<0.05)。研究组妊娠并发症发生率为0,低于对照组的25.00%,差异有统计学意义(P<0.05)。结论妊娠期糖尿病患者采用门冬胰岛素联合地特胰岛素治疗可显著改善血糖指标,且患者并发症发生率较低,可在临床中推广应用。

Objective To analyze the effect of insulin detemir combined with insulin aspart in the treatment of gestational diabetes mellitus and its influence on maternal and infant outcomes.Methods 32 patients with gestational diabetes mellitus admitted to our hospital were selected as the research objects,computer software was used to randomly pick numbers to complete the grouping.The patients with odd numbers were included in the control group(16 cases,treated with insulin aspart alone),and the patients with even numbers were included in the study group(16 cases,treated with insulin aspart combined with insulin detemir).The blood glucose indexes before and after treatment were compared between the two groups,and the incidence of pregnancy complications was recorded.Results Before treatment,there were no significant differences in fasting blood glucose(FPG),2 h postprandial blood glucose(2 h PG)and glycosylated hemoglobin(HbAlc)levels between the two groups(P>0.05);after treatment,the levels of FPG,2 h PG and HbAlc in the two groups were decreased,and the levels of all indexes in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).The incidence of pregnancy complications in the study group was 0,which was lower than 25.00%in the control group,and the difference was statistically significant(P<0.05).Conclusion Insulin aspartate combined with insulin detemir can significantly improve blood glucose index in patients with gestational diabetes mellitus,and the incidence of complications is low,so it can be popularized in clinical application.
