丙硫氧嘧啶联合甲巯咪唑乳膏对妊娠期甲亢患者的激素水平及妊娠结局的影响

Effect of propylthiouracil combined with methimazole cream on hormone level and pregnancy outcome in patients with hyperthyroidism during pregnancy

目的探讨丙硫氧嘧啶联合甲巯咪唑乳膏对妊娠期甲亢患者的激素水平及妊娠结局的影响。方法选择妊娠期甲亢患者80例为研究对象,根据随机数字表法分为对照组与研究组,每组40例。对照组给予丙硫氧嘧啶治疗,研究组给予丙硫氧嘧啶联合甲巯咪唑乳膏。比较两组甲状腺激素水平、不良妊娠结局发生率。结果治疗前,两组甲状腺素(T_(4))、游离甲状腺素(FT_(4))、三碘甲状腺原氨酸(T_(3))、游离三碘甲状腺原氨酸(FT_(3))、促甲状腺激素(TSH)水平比较差异无统计学意义(P>0.05);治疗后,研究组T_(4)、FT_(4)、T_(3)、FT_(3)水平均低于对照组,TSH水平高于对照组,差异均有统计学意义(P<0.05)。研究组总不良妊娠结局发生率低于对照组,差异有统计学意义(P<0.05)。结论丙硫氧嘧啶联合甲巯咪唑乳膏治疗妊娠期甲亢患者,可以有效改善患者的甲状腺激素水平和妊娠结局,值得临床推广与应用。

Objective To investigate the effect of propyl thiouracil combined with methimazole cream on hormone levels and pregnancy outcomes in patients with hyperthyroidism during pregnancy.Methods A total of 80 cases of hyperthyroidism during pregnancy were selected as the research object,and were divided into control group and study group according to the random number table method,40 cases in each group.The control group was treated with propyl thiouracil,and the study group was treated with propyl thiouracil combined with methimazole cream.Thyroid hormone levels and the incidence of adverse pregnancy outcomes were compared between the two groups.Results Before treatment,there was no significant difference in the levels of thyroxine(T_(4)),free thyroxine(FT_(3)),triiodothyronine(T_(3)),free triiodothyronine(FT_(3))and thyrotropin(TSH)between the two groups(P>0.05).After treatment,the levels of T_(4),FT_(4),T_(3) and FT_(3) in the study group were lower than those in the control group,and the levels of TSH in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).The incidence of total adverse pregnancy outcomes in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Propyl thiouracil combined with methimazole cream can effectively improve thyroid hormone levels and pregnancy outcomes in patients with hyperthyroidism during pregnancy,which is worthy of clinical promotion and application.
