程红亮基于“辨经论治”理论治疗中风后吞咽障碍探析

总结程红亮教授基于“辨经论治”理论治疗中风后吞咽障碍的经验。程红亮教授以“辨经”为“论治”的核心,其病位在脑、在咽、与脏腑相关,“脑-咽-脏腑相关”实质是以任督二脉为桥梁,构建起脑-咽-脏腑三者之间的联系。该病的发生是多脏腑功能失调所致,从局部或一脏一经论治是不够的。目前,针灸治疗该病大多集中在局部取穴,经络辨证取穴尚未形成体系。在此基础上,程红亮教授创新性地提出“咽为经脉之所聚”,形成了“辨经论治”治疗吞咽障碍标准化、规范化的理论体系,认为治疗中风后吞咽障碍应根据患者不同的经脉病症特点“辨经选穴”。程红亮主任经过不断的研究探索,确立了以项部取穴+舌咽部取穴+“辨经”取穴的治疗思路,以任督二脉为主,结合芒针透刺,有效提高了临床疗效。

Summarizing Professor CHENG Hongliang’s experience in treating swallowing disorder after stroke based on the theory of “discern the meridians and treatment”.Professor CHENG Hongliang summarizes that “discerning the scriptures” is the core of “treatment”,and its disease position is in the brain,in the pharynx,and related to the internal organs,and the essence of “brain-pharynx-viscera-related” is to use the Ren and Du Pulse as a bridge to build a connection between the brain,pharynx and viscera.Therefore,the occurrence of this disease is caused by the dysfunction of multiple organs,and it is not enough to treat it from the local or one organ.However,at present,most of the acupuncture treatment of this disease is concentrated on local acupuncture,and meridian dialectical acupuncture has not yet formed a system.On this basis,Professor CHENG Hongliang put forward the theory of “the pharynx is the gathering of meridians”,which opened up a precedent for the standardization and standardization of the treatment of swallowing disorders by “discern the meridians and treatment”,and believed that the swallowing disorders after the treatment of stroke should be based on the characteristics of different meridian diseases of patients to “discern the meridians and select the acupuncture points”.After continuous research and exploration,Director CHENG Hongliang established the treatment idea of taking acupuncture points from the neck part + glossopharynx + “discerning the meridians” to take acupuncture points,mainly based on the Ren and Du Pulse,combined with elongated needle,which effectively improved the clinical efficacy.
