摘要
目的了解2020年四川省儿童乙型肝炎(乙肝)表面抗原(Hepatitis B surface antigen,HBsAg)流行率及其影响因素。方法采用分层多阶段整群随机抽样方法抽取四川省1-14岁儿童,采集血清标本,使用化学发光微粒子免疫法测定HBsAg,分析HBsAg阳性率及其影响因素。结果四川省1-14岁儿童HBsAg阳性率为1.65%(16/968);1-4岁、5-9岁、10-14岁儿童分别为0.56%(2/354)、2.26%(7/310)、2.30%(7/304)。多因素Logistic回归分析显示,母亲HBsAg阳性、未测或状态不详的儿童较母亲HBsAg阴性的儿童的HBsAg阳性率高[OR(95%CI):12.94(1.09-153.54)、6.32(1.37-29.19)]。结论2020年四川省1-14岁儿童HBsAg流行率显著下降;需加强HBsAg阳性母亲所生新生儿的乙肝母亲传播阻断措施。
Objective To determine the seroprevalence of hepatitis B surface antigen(HBsAg)and its influencing factors among children aged 1-14 years in Sichuan province in 2020.Methods We used a stratified multi-stage cluster random sampling method to recruit 1-14-year-old children and obtain serum samples.We tested sera for HBsAg using chemiluminescence microparticle immunoassay to analyze the positivity rate of HBsAg and its influencing factors.Results The positivity rate of HBsAg among 1-14-year-old children in Sichuan was 1.65%(16/968).Positivity rates for 1-4-,5-9-,and 10-14-year-olds were 0.56%(2/354),2.26%(7/310),and 2.30%(7/304),respectively.Multivariate logistic regression analysis showed that the HBsAg positivity rate among children who were born to HBsAg-positive mothers and whose mothers did not receive HBsAg testing or did not know their own HBsAg status were higher than among children who were born to HBsAg-negative mothers[OR(95%CI):12.94(1.09-153.54)and 6.32(1.37-29.19)].Conclusions The seroprevalence of HBsAg decreased significantly among 1-14-year-old children in Sichuan in 2020.We need to strengthen interruption of mother-to-child transmission of hepatitis B in newborns of HBsAg-positive mothers.
