杵针对剖宫产产妇缺乳症状及血清泌乳素的影响

Effect of Chuzhen on the symptoms of lactation deficiency and serum prolactin in caesarean section women

目的探讨杵针对剖宫产产妇缺乳症状及血清泌乳素的影响。方法选取2020年8月~2021年2月在成都中医药大学附属医院住院的123例剖宫产缺乳产妇,按照区组随机分组分为观察组61例,对照组62例。对照组实施剖宫产缺乳常规治疗,做好哺乳指导,促乳干预。观察组在对照组的基础上辨证实施杵针技术。每日一次,连续干预5 d,采用重复测量方差分析比较干预前及干预第1、2、3、4、5天后患者的泌乳量、乳房充盈度、乳汁淤积程度、乳汁粘稠度评分。比较干预前后两组产妇的血清泌乳素。结果不同时间点的泌乳量评分组间比较:干预前2天观察组与对照组相比无差异统计学意义(P>0.05),第3天起组间比较差异有统计学意义(P<0.05);不同时间点乳房充盈度评分的组间比较:干预前1天观察组与对照组相比差异无统计学意义(P>0.05),第2天起组间比较差异有统计学意义(P<0.05);不同时间点的乳汁淤积度评分组间比较:干预的前2天观察组与对照组相比差异无统计学意义(P>0.05),第3天起组间比较差异有统计学意义(P<0.05);不同时间点的乳汁黏稠度评分组间比较:干预前3天观察组与对照组相比差异无统计学意义(P>0.05),第4天起组间比较差异有统计学意义(P<0.05)。两组的泌乳量、乳房充盈度、乳汁淤积程度、乳汁粘稠度评分四项指标的组间效应、时间效应及组间与时间的交互效应均有统计学意义(P<0.05)。干预后观察组血清泌乳素高于对照组(P<0.05)。结论杵针可提高剖宫产后缺乳产妇的泌乳量,增加乳房充盈度,改善乳汁淤积程度,增加乳汁粘稠度,促进血清泌乳素分泌。

Objective To explore the effect of Chuzhen on lactation symptoms and Serum Prolactin of Cesarean Section women.Methods A total of 123 cases of pregnant women with lactation after cesarean section in The Affiliated Hospital of Chengdu University of TCM from August 2020 to February 2021 were randomly divided into the control group(61 cases)and the study group(62 cases).The control group was treated with routine treatment.The test group implemented the Chuzhen on the basis of evidence in the control group.Once a day for 5 consecutive interventions,Using RMANOVA to compare the milk yield,breast filling,milk deposition and milk viscosity of patients before intervention and after 1,2,3,4 and 5 days of intervention.The Serum lactogen was compared between the two groups before and after the intervention.Results Comparison of the scores of lactation symptoms:there was no significant difference between the test group and the control group at 2 days before intervention(P>0.05),but there was significant difference between the two groups from the third day(P<0.05).The intra-group comparison of breast filling at different time points showed that there was no significant difference between the experimental group and the control group on the first day before intervention(P>0.05),but there was significant difference in the second day(P<0.05).The intra-group comparison of milk siltation degree at different time points showed that there was no significant difference between the experimental group and the control group in the first 2 days of intervention(P>0.05),but there was significant difference in the third day group(P<0.05).The intra-group comparison of milk viscosity at different time points showed that there was no significant difference between the experimental group and the control group 3 days before intervention(P>0.05),but there was significant difference between the experimental group and the control group from the 4th day(P<0.05).The intergroup effect,time effect and intergroup and time interaction effect of milk yield,breast filling degree,milk deposition degree and milk viscosity in both groups were statistically significant(P<0.05).After intervention,serum prolactin in experimental group was higher than that in control group(P<0.05).Conclusion Chuzhen can improve milk yield,increase breast fullness,improve the degree of milk stasis,increase milk consistency,and promote serum lactogen secretion in mothers with post cesarean deficiency.
