醋酸戈舍瑞林联合腹腔镜手术治疗对子宫内膜异位症患者的应用价值

Application value of goserelin acetate combined with laparoscopic surgery in the treatment of endometriosis of patients

目的分析醋酸戈舍瑞林联合腹腔镜手术治疗对子宫内膜异位症(EMT)患者的临床疗效、卵巢功能及妊娠率的影响。方法收集2016年1月~2020年12月我院收治的110例EMT患者的临床资料。按照不同治疗方法分为单纯组(n=54例,腹腔镜手术)与联合组(n=56例,醋酸戈舍瑞林联合腹腔镜手术)。比较两组临床疗效、卵巢功能、排卵、月经恢复时间、妊娠率、输卵管畅通率、复发率及不良反应发生情况。结果联合组治疗后总有效率高于单独组(P<0.05)。治疗后,两组血清卵泡刺激素(FSH)、促黄体生成素(LH)及雌二醇(E_(2))水平均较治疗前明显下降,联合组上述因子水平下降更为明显(P<0.05)。联合组患者排卵恢复时间较单纯组短;月经恢复时间较单纯组长(P<0.05)。联合组妊娠率、输卵管畅通率高于单纯组,复发率、不良反应总发生率低于单纯组(P<0.05)。结论醋酸戈舍瑞林联合腹腔镜手术治疗EMT患者疗效确切,可有效改善患者卵巢功能,提高妊娠率,降低复发率。

Objective To analyze the application value of goserelin acetate combined with laparoscopic surgery in the treatment of endometriosis(EMT)of patients.Methods The clinical data of 110 EMT patients admitted to this hospital from January 2016 to December2020 were collected.According to different treatment methods,they were divided into simple group(n=54 cases,laparoscopic surgery)and combined group(n=56 cases,goserelin acetate combined with laparoscopic surgery).The clinical efficacy,ovarian function,ovulation,menstrual recovery time,pregnancy rate,patency rate of fallopian tube,recurrence rate and adverse reactions were compared between the two groups.Results The total effective rate after treatment in the single group was 83.33%,and that in the combined group was 96.43%,which was significantly higher than that in the control group(P<0.05).After treatment,the levels of serum follicle stimulating hormone(FSH),luteinizing hormone(LH)and estradiol(E2)in the two groups were significantly lower than those before treatment.The levels of the above factors decreased more significantly in the combined group(P<0.05).The recovery time of ovulation in the combined group were shorter than those in the simple group,and the menstrual recovery time was longer than that of the simple group(P<0.05).The pregnancy rate and patency rate of fallopian tube of the combined group were significantly higher than those of the simple group,and the recurrence rate and the total incidence of adverse reactions were lower than those of the simple group(P<0.05).Conclusion Goserelin acetate combined with laparoscopic surgery has a definite effect in the treatment of EMT patients,which can effectively improve the patient's ovarian function,increase pregnancy rate,and reduce recurrence rate.
