结直肠癌患者术前血液指标与临床分期及预后的相关性

Relationship between preoperative coagulation indexes and clinical stage and prognosis in patients with colorectal cancer

目的探讨结直肠癌患者术前凝血指标和淋巴细胞与单核细胞比值(LMR)与临床分期及预后的相关性。方法选取2018年2月至2020年10月间安康市紫阳县中医医院和西安市第九医院诊治的80例结直肠癌患者作为结直肠癌组,同期选择体检的80例健康人群作为对照组,检测与计算D-二聚体与LMR,随访结直肠癌的预后并进行相关性分析。结果结直肠癌组的LMR低于对照组,D-二聚体高于对照组,差异均有统计学意义(均P<0.05)。在结直肠癌组中,Spearsman相关分析显示,T分期、N分期、M分期与LMR、D-二聚体存在相关性,差异均有统计学意义(均P<0.05)。随访到2021年10月1日,无进展生存时间为(17.82±1.33)个月;因素Cox比例风险模型显示,LMR、D-二聚体为影响患者无进展生存时间的重要因素,差异均有统计学意义(均P<0.05)。结论结直肠癌患者多伴随D-二聚体升高与LMR下降,与患者的临床TNM分期存在相关性,是影响结直肠癌患者预后的重要因素。

Objective To investigate the relationship between preoperative coagulation indexes and lymphocyte-to-monocyte ratio(LMR)and clinical stage and prognosis in patients with colorectal cancer.Methods From February 2018 to October 2020,80 patients with colorectal cancer diagnosed and treated at Ziyang County Hospital of Traditional Chinese Medicine and Xi’an Ninth Hospital were selected and included in a colorectal cancer group,and 80 healthy individuals who received physical examination at the same hospital during the same period were selected and include in a control group.D-dimer were detected and calculated the LMR,prognosis of colorectal cancer was followed-up and correlation was analyzed.Results The LMR were lower in the colorectal cancer group than in the control group(P<0.05),and the D-dimer were higher than in the control group(all P<0.05).In the colorectal cancer group,Spearsman correlation analysis showed that T stage,N stage and M stage were correlated with LMR and D-dimer(all P<0.05).The participants were followed-up up to October 1,2021,and progression-free survival was 17.82±1.33 months.Cox proportional hazard model showed that LMR and D-dimer were important factors influencing progression-free survival(all P<0.05).Conclusion Colorectal cancer are mostly accompanied by increased D-dimer and decreased LMR,which are related to the clinical TNM staging of colorectal cancer.LMR and D-dimer are important factors influencing the prognosis of patients with colorectal cancer.
