地佐辛合舒芬太尼在腹腔镜胃癌手术中的镇痛效果观察

Analgesic effect of dezocine combined with sufentanil in patients undergoing laparoscopic surgery for gastriccancer

目的探讨地佐辛复合舒芬太尼对腹腔镜胃癌手术患者应激水平、致痛物质以及镇痛、镇静的影响。方法选取2019年1月至2021年12月间上海市公共卫生临床中心收治的82例腹腔镜胃癌手术患者,根据随机数表法分为对照组和观察组,每组41例。对照组采用舒芬太尼镇痛,观察组采用地佐辛复合舒芬太尼镇痛,比较术后各时间段致痛物质水平、应激反应水平以及视觉模拟疼痛(VAS)评分和躁动率。结果观察组术后24h、48h的P物质、5-羟色胺和超氧化物歧化酶水平低于对照组,丙二醛水平高于对照组,差异均有统计学意义(均P<0.05)。同时观察组术后6h时VAS评分(4.18±1.65)分、术后24hVAS评分(2.16±0.32)分、术后48hVAS评分(1.78±0.32)分以及术后6h、24h、48h的躁动率均低于对照组,差异均有统计学意义(均P<0.05)。结论在腹腔镜胃癌术后实施地佐辛复合舒芬太尼干预,可发挥显著镇痛、镇静效果。

Objective To compare the effects of dezocine and sufentanil on stress response,pain causing substances,analgesia and sedation in patients undergoing laparoscopic surgery for gastric cancer.Methods Eighty-two patients with laparoscopic gastric cancer were treated prospectively from January 2019 to December 2021.They were randomly divided into an observation group and a control group with 41 patients in each group.The observation group was given dezocine combined with sufentanil and the control group was given sufentanil alone for analgesia.The levels of pain causing substances,stress response,visual analog pain(VAS)score and agitation rate at each time after operation were compared.Results the levels of PS,5-hydroxytryptamine(5-HT)and super oxide dismutase(SOD)were lower in the observation group at 24 h and 48 h after the operation than those in the control group,and the level of malondialdehyde(MDA)was higher than that in the control group(all P<0.05).Moreover,VAS score at 6 h(4.18±1.65),24 h(2.16±0.32)and 48 h(1.78±0.32)and the restlessness rate at 6 h,24 h and 48 h were lower in the observation group than those in the control group(all P<0.05).ConclusionDezocine combined with sufentanil can play a significant analgesic and sedative effect after laparoscopic surgery for gastric cancer.
