MSCT对肝脏局灶性结节增生与肝细胞腺瘤的诊断及鉴别诊断

Diagnosis and Differential Diagnosis of Focal Nodular Hyperplasia and Hepatocellular Adenoma by MSCT

目的:探讨MSCT多期扫描在肝脏局灶性结节增生与肝细胞腺瘤的诊断和鉴别诊断中的价值。方法:回顾性分析经手术病理证实的32例局灶性结节增生(focal nodular hyperplasia,FNH)和15例肝细胞腺瘤(hepatocellular adenoma,HCA)的CT多期扫描资料,分别测量各期病灶的CT值,并进行统计学分析。结果:32例FNH共35个病灶,平扫呈等或稍低密度,动脉期明显强化,门脉期及实质期呈稍高或等密度,32例FNH不同时期平均CT值差异有统计学意义;15例HCA共22个病灶,平扫呈等或稍低密度,动脉期较明显强化,门脉期呈稍高密度,实质期呈稍高或等密度,15例HCA不同时期平均CT值差异有统计学意义;FNH与HCA在动脉期的平均CT值差异有统计学意义,其余各期CT值差异无统计学意义。结论:MSCT多期扫描分析有助于FNH与HCA的诊断及鉴别诊断。

Objective:To investigate the value of multi-phase MSCT scanning in the diagnosis and differential diagnosis of focal nodular hyperplasia and hepatocellular adenoma of the liver.Methods:The multiphase CT data of 32 cases of focal nodular hyperplasia(FNH)and 15 cases of hepatocellular adenoma(HCA)confirmed by surgery and pathology were retrospectively analyzed.The CT values of each stage were measured.Statistical analysis was performed.Results:There were 35 lesions in 32 cases of FNH.The plain scan showed equal or slightly low density,the arterial phase was significantly enhanced,and the portal and parenchyma phases showed slightly high or equal density.The average CT values of 32 cases of FNH in different periods were statistically significant;There were 22 lesions in 15 cases of HCA,which showed equal or slightly low density on plain scan,significantly enhanced in arterial phase,slightly high density in portal phase,and slightly high or equal density in parenchyma phase.The average CT values of 15 cases of HCA in different periods were statistically significant;The average CT values of FNH and HCA in arterial phase were statistically significant,while the CT values in other phases were not statistically significant.Conclusion:MSCT multi-phase scan analysis is helpful in the diagnosis and differential diagnosis of FNH and HCA.
