无创呼吸支持对合并呼吸窘迫综合征早期早产儿纠正胎龄40周肺功能的影响 被引量:2

Effect of noninvasive ventilation on pulmonary function of early preterm infants with respiratory distress syndrome after correcting gestational age of 40 weeks

摘要 目的比较两种无创呼吸支持模式对早期早产儿纠正胎龄40周潮气呼吸肺功能的影响。方法纳入入住新生儿科符合研究标准的早产儿60例,随机分为无创正压通气(noninvasive positive pressure ventilation,NIPPV)组及持续气道正压通气(continuous positive airway pressure,CPAP)组。收集两组患儿基础临床资料、血气及肺功能检查结果进行分析。结果两组基础临床资料、早产儿相关并发症差异无统计学意义(P>0.05);NIPPV组需机械通气较CPAP组少,但差异无统计学意义(P>0.05),两组血气pH值、动脉血二氧化碳分压(PCO_(2))、剩余碱(BE)随时间而变化,但组间差异及组间随时间变化差异无统计学意义(P>0.05);两组纠正胎龄40周肺功能监测差异无统计学意义(P>0.05)。结论NIPPV及CPAP通气方式均可改善新生儿呼吸窘迫综合征早产儿血气,对早期早产儿纠正胎龄40周肺功能影响无差异,临床应重视早期早产儿肺功能的监测。 Objective To analyze the effect of two noninvasive respiratory support modes on tidal breath and pulmonary function of early preterm infants corrected for 40 weeks of gestational age.Methods This study included premature infants who met the inclusion criteriain neonatal department.They were randomly divided into NIPPV group and CPAP group according to the needs of noninvasive ventilation.The basic clinical data,blood gas and pulmonary function examination results of the two groups were collected and analyzed.Results There was no significant difference in basic clinical data and related complications between the two groups.The NIPPV group needed less mechanical ventilation than the CPAP group,though the difference was not statistically significant.There was no significant difference in pH,PCO_(2) or BE between the 2 groups.There was no significant difference in pulmonary function monitoring between the two groups after 40 weeks of gestational age.Conclusion Noninvasive NIPPV and CPAP ventilation can both improve the blood gas of preterm infants with neonatal respiratory distress syndrome,and there is no significant difference in the effect of correcting the pulmonary function and complications of early preterm infants at 40 weeks of gestational age.Clinical attention should be paid to the monitoring of pulmonary function of early preterm infants.
