超声引导下腹横肌平面阻滞对腹腔镜结肠癌手术患者的临床疗效分析

Clinical Efficacy Analysis of Ultrasound-Guided Transversus Abdominis Plane Block in Patients with Laparoscopic Colon Cancer Surgery

目的:分析腹腔镜结肠癌手术患者行超声引导下腹横肌平面阻滞对其免疫功能及应激反应的影响。方法:选取2020年1月-2022年1月江苏省如皋市博爱医院行腹腔镜结肠癌手术治疗的患者60例为观察对象,根据麻醉方式不同将其分为对照组与研究组,各30例。对照组给予全身麻醉,研究组给予全麻复合超声引导下腹横肌平面阻滞。对比两组患者麻醉前后的免疫功能、应激反应及血流动力学指标。结果:麻醉后,研究组CD_(4)^(+)水平高于对照组,肾上腺素、去甲肾上腺素、CD_(8)^(+)水平低于对照组,差异有统计学意义(P<0.05);麻醉后,研究组的心率、平均动脉压水平低于对照组,差异有统计学意义(P<0.05)。结论:超声引导下腹横肌平面阻滞对腹腔镜结肠癌手术患者应激反应及免疫功能影响较小,且能保证血流动力学稳定。

Objective:To analyze the influence of ultrasound-guided transversus abdominis plane block on immune function and stress response in patients undergoing laparoscopic colon cancer surgery.Methods:A total of 60 patients who underwent laparoscopic colon cancer surgery in Bo'ai Hospital of Rugao City of Jiangsu Province from January 2020 to January 2022 were selected as the observation objects.The patients were divided into control group and study group according to the anesthesia method,with 30 cases in each group.The control group underwent general anesthesia,and the study group underwent general anesthesia combined with ultrasound-guided transversus abdominis plane block.The immune function,stress response and hemodynamic indexes before and after anesthesia were compared between two groups.Results:After anesthesia,the levels of CD_(4)^(+)in the study group were higher than those in the control group,while epinephrine,norepinephrine and CD_(8)^(+)levels were lower than those in the control group,and the differences were statistically significant(P<0.05).After anesthesia,the levels of heart rate and mean arterial pressure in the study group were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:Ultrasound-guided transversus abdominis plane block has little effect on the stress response and immune function of patients undergoing laparoscopic colon cancer surgery.And it can ensure hemodynamic stability.
