降钙素原和白细胞介素6在新生儿肺炎早期诊断中的应用价值

摘要 目的:探讨降钙素原(PCT)和白细胞介素6(IL-6)在新生儿肺炎早期诊断中的应用价值。方法:选取2018年1月-2021年4月烟台业达医院收治的新生儿细菌感染性肺炎患儿50例为研究组;选取同阶段健康新生儿50例为参照1组;选取同阶段收治的新生儿非细菌感染性肺炎患儿50例为参照2组。检验并比较三组PCT、IL-6、红细胞沉降率(ESR)及超敏C反应蛋白(hs-CRP)指标水平。研究组患儿依据细菌感染性肺炎疾病程度分为重症组(20例)及轻症组(30例),将两组PCT、IL-6水平同参照1组对比。结果:研究组PCT、IL-6水平高于参照1组与参照2组,差异有统计学意义(P<0.05);研究组、参照1组、参照2组ESR、hs-CRP水平比较,差异无统计学意义(P>0.05)。重症组PCT、IL-6水平均高于轻症组与参照1组,差异有统计学意义(P<0.05)。结论:新生儿肺炎患儿同健康新生儿比较,其PCT及IL-6水平显著提高,临床检测PCT及IL-6水平,可为新生儿肺炎疾病早期确诊提供有力依据,实现新生儿肺炎的及时治疗,促进其良好预后。 Objective:To investigate the application value of procalcitonin(PCT)and interleukin-6(IL-6)in the early diagnosis of neonatal pneumonia.Methods:A total of 50 children with neonatal bacterial pneumonia who were admitted to Yantai Yeda Hospital from January 2018 to April 2021 were selected as the study group.At the same stage,50 healthy newborns were selected as the reference group 1.At the same stage,50 children with neonatal non-bacterial pneumonia were selected as the reference group 2.PCT,IL-6,erythrocyte sedimentation rate(ESR)and hypersensitive C-reactive protein(hs-CRP)were detected and compared among three groups.The children with bacterial pneumonia were further divided into severe group(n=20)and mild group(n=30)according to the severity of bacterial pneumonia,and the PCT level and IL-6 level of these two groups were compared with those of the reference group 1.Results:The levels of PCT and IL-6 in the study group were higher than those in the reference groups 1 and 2,and the difference was statistically significant(P<0.05).There were no significant differences in ESR and hs-CRP levels among three groups(P>0.05).The levels of PCT and IL-6 in the severe group were higher than those in mild group and reference group 1,and the difference was statistically significant(P<0.05).Conclusion:Compared to the healthy newborns,the PCT and IL-6 levels in children with neonatal pneumonia are significantly increased.The active implementation of PCT and IL-6 detection in clinical practice can provide strong basis for early diagnosis of neonatal pneumonia disease,achieve the timely treatment of neonatal pneumonia,and promote good prognosis.
