多层螺旋CT图像后处理技术诊断气管支气管结核的效能分析

Analysis of the Efficacy of Multilayer Spiral CT Image Post-Processing Technique for the Diagnosis of Tracheobron⁃chial Tuberculosis

摘要 目的:探讨多层螺旋CT(MSCT)图像后处理技术诊断气管支气管结核的效能。方法:选取2019年8月-2021年11月红安县中医医院收治的气管支气管结核患者102例为研究对象,所有患者行MSCT检查(MSCT组)和电子纤维支气管镜检查(纤维镜组),分析MSCT图像后处理技术诊断气管支气管结核的价值。结果:通过病理检查出气管支气管结核患者52例,非气管支气管结核患者50例,经MSCT图像后处理技术检查为气管支气管结核患者48例,非气管支气管结核患者54例,以电子纤维支气管镜病理检查结果为金标准,其诊断灵敏度为86.54%,特异度为94.00%,准确度为90.20%,Kappa值为0.804,一致性高;多层螺旋CT图像后处理技术检查支气管管壁增厚、管腔狭窄和肺内结核灶检出率与电子纤维支气管镜病理检查结果比较,差异无统计学意义(P>0.05)。结论:MSCT图像后处理技术在检查气管支气管结核方面准确性较高,可精确诊断气管支气管结核,灵敏度、特异度及准确度高。 Objective:To investigate the efficacy of multilayer spiral CT(MSCT)image post-processing technique for the diagno⁃sis of tracheobronchial tuberculosis.Methods:102 patients with tracheobronchial tuberculosis admitted to Hongan County Hospi⁃tal of Traditional Chinese Medicine from August 2019 to November 2021 were selected for the study.All patients underwent MSCT examination(MSCT group)and electronic fiberoptic bronchoscopy(fiberoptic group)to analyze the value of MSCT image post-processing techniques for diagnosing tracheobronchial tuberculosis.Results:52 patients with tracheobronchial tuberculosis and 50 patients with non-tracheobronchial tuberculosis were detected by pathological examination.There were 48 patients with tracheobronchial tuberculosis and 54 patients with non-tracheobronchial tuberculosis examined by MSCT image post-processing technique.The diagnostic sensitivity was 86.54%,the specificity was 94.00%,the accuracy was 90.20%,and the Kappa value was 0.804 with high concordance,using the electronic fiberoptic bronchoscopy pathological examination results as the gold standard.There was no statistically significant difference between the detection rate of bronchial wall thickening,luminal stenosis and intrapulmonary tuberculosis foci by multilayer spiral CT image post-processing technique and the pathological findings by electronic fiberoptic bronchoscopy(P>0.05).Conclusion:MSCT image post-processing technique is more accurate in the examination of tra⁃cheobronchial tuberculosis and can accurately diagnose tracheobronchial tuberculosis with high sensitivity,specificity and accu⁃racy.
