糖皮质激素联合过敏煎对哮喘患者炎症反应及呼出气一氧化氮水平的影响

Effect of Glucocorticoid Combined with Guomin Jian on Inflammatory Responses and Exhaled Nitric Oxide Levels of Patients with Asthma

目的:观察糖皮质激素联合过敏煎对哮喘患者炎症反应及呼出气一氧化氮(FeNO)水平的影响。方法:选取92例哮喘患者,按随机数字表法分为对照组及观察组各46例。对照组予以糖皮质激素治疗,观察组在对照组基础上加用过敏煎口服,2组均持续治疗2周。比较2组临床疗效,比较2组治疗前后肺功能指标[第1秒呼吸容积(FEV1)、用力肺活量(FVC)、最高呼气流速(PEF)]、血清因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、超敏C-反应蛋白(hs-CRP)]及FeNO水平的变化,使用Pearson分析FeNO水平与炎症指标相关性。结果:观察组临床疗效总有效率为95.65%,对照组为80.43%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组FEV1、FVC、PEF水平均较治疗前上升(P<0.05),观察组上述3项指标值均高于对照组(P<0.05)。治疗后,2组TNF-α、IL-6、IL-8、hs-CRP水平均较治疗前下降(P<0.05),观察组上述4项水平均低于对照组(P<0.05)。治疗后,2组FeNO水平均较治疗前下降(P<0.05),观察组低于对照组(P<0.05)。经Pearson相关性分析,FeNO水平与TNF-α、IL-6、IL-8及hs-CRP均具有相关性,以IL-6相关程度最高(r=0.663),hs-CRP相关程度最低(r=0.312)。结论:糖皮质激素联合过敏煎治疗哮喘疗效确切,肺功能增强,炎症反应改善,且FeNO水平有所降低。

Objective:To observe the effect of the therapy of glucocorticoid combined with Guomin Jian on inflammatory responses and levels of exhaled nitric oxide(FeNO)in patients with asthma.Methods:A total of 92 cases of patients with asthma were divided into the control group and the observation group according to the random number table method,with 46 cases in each group.The control group was treated with glucocorticoid,and the observation group was additionally treated with Guomin Jian based on the treatment of the control group.Both groups were continuously treated for two weeks.The clinical effects were compared between the two groups.Before and after treatment,the lung function indexes,including forced expiratory volume in one second(FEV1),forced vital capacity(FVC)and peak expiratory flow(PEF),serum factors,including the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-8(IL-8)and high-sensitivity C-reactive protein(hs-CRP),and Fe NO were compared between the two groups.Pearson was used to analyze the correlation between FeNO levels and inflammatory indexes.Results:The total clinical effective rate was 95.65% in the observation group and 80.43% in the control group,the difference being significant(P<0.05).After treatment,the levels of FEV1,FVC and PEF in the two groups were increased when compared with those before treatment(P<0.05),and the above three levels in the observation group were higher than those in the control group(P<0.05).After treatment,the levels of TNF-α,IL-6,IL-8 and hs-CRP in the two groups were decreased when compared with those before treatment(P<0.05),and the above four levels in the observation group were lower than those in the control group(P<0.05).After treatment,the FeNO levels in the two groups were decreased when compared with those before treatment(P<0.05),and the FeNO level in the observation group was lower than that in the control group(P<0.05).Pearson correlation analysis showed that there were correlations between the FeNO level and TNF-α,IL-6,IL-8 and hs-CRP,with the strongest between FeNO and IL-6(r=0.663)and the lowest between FeNO and hs-CRP(r=0.312).Conclusion:The therapy of glucocorticoid combined with Guomin Jian has a definite curative effect on asthma,which can enhance the lung function,improve the inflammatory responses,and reduce the Fe NO level.
