驱寒除湿方烫熨结合枕颌牵引治疗神经根颈椎病风寒湿证临床研究

Clinical Study on Hot Medicated Compress with Quhan Chushi Prescription Combined with Occipito-Mandibular Traction for Cervical Spondylotic Radiculopathy with Wind-Cold-Dampness Syndrome

目的:观察驱寒除湿方烫熨结合枕颌牵引治疗神经根颈椎病风寒湿证的效果。方法:将104例神经根颈椎病风寒湿证患者,按随机数字表法分为对照组与观察组各52例。对照组通过枕颌牵引治疗,观察组通过驱寒除湿方烫熨结合枕颌牵引治疗。比较2组临床疗效,比较2组治疗前后中医证候积分、视觉模拟评分法(VAS)评分、颈椎病临床评价量表(CASCS)评分、颈椎活动度等。结果:观察组总恢复率98.08%,高于对照组86.54%(P<0.05)。与同组治疗前比较,治疗后2组颈肩肢痛、上肢麻木、颈项僵硬、上肢沉重、活动受限各项中医证候积分及总分均降低(P<0.05);与对照组治疗后比较,观察组治疗后各项中医证候积分及总分均较低(P<0.05)。与同组治疗前比较,治疗后2组VAS评分均降低(P<0.05),CASCS评分均升高(P<0.05);与对照组治疗后比较,观察组治疗后VAS评分较低(P<0.05),CASCS评分较高(P<0.05)。与同组治疗前比较,治疗后2组颈椎屈曲度、颈椎侧屈度、颈椎伸展度、颈椎旋转度均增大(P<0.05);与对照组治疗后比较,观察组治疗后颈椎屈曲度、颈椎侧屈度、颈椎伸展度、颈椎旋转度均较大(P<0.05)。结论:驱寒除湿方烫熨结合枕颌牵引可显著缓解神经根颈椎病风寒湿证患者的临床症状,减轻患者痛苦,恢复正常的颈椎结构与功能,提高恢复率。

Objective:To observe the effect of hot medicated compress with Quhan Chushi Prescription combined with occipito-mandibular traction for cervical spondylotic radiculopathy with wind-cold-dampness syndrome.Methods:A total of 104 cases of patients with cervical spondylotic radiculopathy with windcold-dampness syndrome were divided into the control group and the observation group according to the random number table method,with 52 cases in each group.The control group was treated with occipitomandibular traction,and the observation group was treated with hot medicated compress with Quhan Chushi Prescription combined with occipito-mandibular traction.The clinical effects in the two groups were compared.Before and after treatment,Chinese medicine syndrome scores,the scores of Visual Analogue Scale(VAS)and Clinical Assessment Scale for Cervical Spondylosis(CASCS),and cervical range of motion in the two groups were compared.Results:The total recovery rate was 98.08% in the observation group,higher than that of 86.54% in the control group(P<0.05).After treatment,the scores of various traditional Chinese medicine(TCM)syndromes including pain in neck,shoulder and limbs,numbness in upper limbs,stiffness in neck,heaviness in upper limbs and activity limitation as well as the total scores in the two groups were decreased when compared with those before treatment(P<0.05);the scores of various TCM syndromes and the total score in the observation group were lower than those in the control group(P<0.05).After treatment,the scores of VAS in the two groups were decreased when compared with those before treatment(P<0.05),and the scores of CASCS were increased(P<0.05);the scores of VAS in the observation group was lower than those in the control group(P<0.05),and the scores of CASCS was higher(P<0.05).After treatment,the degrees of cervical flexion,cervical lateral flexion,cervical extension and cervical rotation in the two groups were increased when compared with those before treatment(P<0.05);the degrees of cervical flexion,cervical lateral flexion,cervical extension and cervical rotation in the observation group were higher than those in the control group(P<0.05).Conclusion:The therapy of hot medicated compress with Quhan Chushi Prescription combined with occipito-mandibular traction for patients with cervical spondylotic radiculopathy with wind-cold-dampness syndrome can significantly relieve their clinical symptoms,alleviate their pain,restore the normal structure and function of cervical spine,and improve the recovery rate.
