温经散寒活血方联合妇女养血丸合红金消结胶囊治疗子宫肌瘤寒凝血瘀证临床研究

Clinical Study on Wenjing Sanhan Huoxue Prescription Combined with Funv Yangxue Pills and Hongjin Xiaojie Capsules for Hysteromyoma with Cold Accumulation Causing Blood Stasis Syndrome

目的:观察温经散寒活血方联合妇女养血丸合红金消结胶囊治疗子宫肌瘤寒凝血瘀证的临床疗效。方法:选取子宫肌瘤寒凝血瘀症患者113例,按随机数字表法分为研究组57例和对照组56例。对照组给予妇女养血丸合红金消结胶囊治疗,研究组在对照组基础上给予温经散寒活血方口服。评价2组临床疗效,比较2组治疗前后性激素水平、中医证候评分、血流动力学及血液流变学指标水平变化,检测治疗前后子宫肌瘤体积、子宫体积大小与月经量变化。结果:研究组总有效率为92.98%,高于对照组80.36%(P<0.05)。治疗后,2组卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)及孕酮(P)水平较治疗前降低(P<0.05),且研究组FSH、LH、E_(2)及P水平均低于对照组(P<0.05)。治疗后,2组各项中医证候评分较治疗前降低(P<0.05),且研究组各项中医证候评分均低于对照组(P<0.05)。治疗后,2组血流灌注指数(PI)、血流阻力指数(RI)、红细胞比容(Hct)较治疗前升高,纤维蛋白原(FIB)较治疗前降低(P<0.05);且研究组PI、RI、Hct高于对照组,FIB低于对照组(P<0.05)。治疗后,2组子宫体积、肌瘤体积及月经量较治疗前降低(P<0.05),且研究组子宫体积、肌瘤体积及月经量均低于对照组(P<0.05)。结论:温经散寒活血方联合妇女养血丸合红金胶囊治疗子宫肌瘤寒凝血瘀证可有效提高临床疗效,改善患者临床症状,调节血清性激素的表达,改善血流动力学及血液流变学指标,缩小子宫肌瘤体积。

Objective:To observe the clinical effect of Wenjing Sanhan Huoxue Prescription combined with Funv Yangxue Pills and Hongjin Xiaojie Capsules for hysteromyoma with cold accumlation causing blood stasis syndrome.Methods:A total of 113 cases of patients with hysteromyoma with cold accumulation causing blood stasis syndrome were selected and divided into the study group and the control group according to the random number table method,with 57 and 56 cases in each group respectively.The control group was treated with Funv Yangxue Pills and Hongjin Xiaojie Capsules,and the study group was additionally given oral administration of Wenjing Sanhan Huoxue Prescription based on the treatment of the control group.The clinical effects in the two groups were evaluated.Before and after treatment,the changes in sex hormones levels,Chinese medicine syndrome scores,and levels of hemodynamics and hemorrheology indexes were compared between the two groups.Before and after treatment,the changes in uterine fibroid volume,uterine volume and menstrual volume were detected.Results:The total effective rate was 92.98% in the study group,higher than that of 80.36% in the control group(P<0.05).After treatment,the levels of follicle stimulating hormone(FSH),luteinizing hormone(LH),estrogen(E2)and progesterone(P)in the two groups were decreased when compared with those before treatment(P<0.05),and the levels of FSH,LH,E_(2) and P in the study group were lower than those in the control group(P<0.05).After treatment,the Chinese medicine syndrome scores in the two groups were decreased when compared with those before treatment(P<0.05),and the Chinese medicine syndrome scores in the study group were lower than those in the control group(P<0.05).After treatment,pulsatility indexes(PI),resistance index(RI)and hematocrit(Hct)in the two groups were increased when compared with those before treatment,and the fibrinogen(FIB)levels were decreased(P<0.05);the levels of PI,RI and Hct in the study group were higher than those in the control group,and the FIB level was lower(P<0.05).After treatment,the uterine volume,fibroid volume and menstrual volume in the two groups were decreased when compared with those before treatment(P<0.05),and the uterine volume,fibroid volume and menstrual volume in the study group were lower than those in the control group(P<0.05).Conclusion:Wenjing Sanhan Huoxue Prescription combined with Funv Yangxue Pills and Hongjin Xiaojie Capsules for hysteromyoma with cold accumulation causing blood stasis syndrome can effectively improve clinical effects,clinical symptoms and hemodynamics and hemorrheology indexes,regulate the expressions of sex hormones in serum and reduce the fibroid volume.
