鼻内镜和微型吸切器联合支撑喉镜手术治疗声带息肉患者的效果

Effects of Nasal Endoscopy + microdebrider + suspension laryngoscope in treatment of vocal cord polyps

目的:观察鼻内镜和微型吸切器联合支撑喉镜手术治疗声带息肉患者的效果。方法:选取2018年1月至2020年6月该院收治的100例声带息肉患者进行前瞻性研究,按照随机数字表法分为对照组和观察组各50例。对照组实施常规支撑喉镜术治疗,观察组实施鼻内镜和微型吸切器联合支撑喉镜手术治疗,比较两组临床疗效、预后情况、治疗前后嗓音障碍指数量表(VHI)评分及并发症发生率。结果:观察组治疗总有效率为98.00%,高于对照组的84.00%,差异有统计学意义(P<0.05);观察组首次发声时间、声音完全恢复时间及住院时间均短于对照组,差异有统计学意义(P<0.05);观察组功能方面、生理方面及情感方面等VHI评分均低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为4.00%,低于对照组的16.00%,差异有统计学意义(P<0.05)。结论:鼻内镜和微型吸切器联合支撑喉镜手术治疗声带息肉患者,可提高临床疗效,缩短预后时间,改善嗓音障碍,降低并发症发生率,其效果优于常规支撑喉镜手术治疗。

Objective: To observe effects of Nasal Endoscopy + microdebrider + suspension laryngoscope in treatment of vocal cord polyps.Methods: A prospective study was conducted on 100 patients with vocal cord polyps admitted to this hospital from January 2018 to June 2020. They were divided into control group and observation group according to the random number table method, 50 cases in each group. The control group was treated with conventional suspension laryngoscope, while the observation group was treated with nasal endoscopy + microdebrider + suspension laryngoscope. The clinical efficacy, the prognosis, the voice handicap index(VHI) score before and after the treatment, and the incidence of complications were compared between the two groups. Results: The total effective rate of treatment in the observation group was 98.00%, which was higher than 84.00% in the control group, and the difference was statistically significant(P<0.05). The first vocalization time, the complete recovery time and the hospitalization time of the observation group were shorter than those of the control group, and the differences were statistically significant(P<0.05). The VHI scores of functional, physiological and emotional aspects in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05). Further, the incidence of complications in the observation group was 4.00%, which was lower than 16.00% in the control group, and the difference was statistically significant(P<0.05). Conclusions: Nasal endoscopy + microdebrider +suspension laryngoscope for the patients with vocal cord polyps can improve clinical efficacy, shorten the prognosis time, improve the voice disorders,and reduce the incidence of complications. Moreover, it is superior conventional suspension laryngoscope surgery.
