抗苗勒管激素在青春期启动女童性发育进程中的表达和意义

摘要 目的 检测青春期启动女童不同性发育进程中的抗苗勒管激素(AMH)水平,分析其变化及与性发育指标的关系,评价AMH的检测意义。方法 选择2016年6月-2019年12月江苏省苏北人民医院儿科内分泌专病门诊收治的996例女童为研究对象,收集各阶段女童的血清AMH浓度及性发育指标的数据。结果 各发育阶段女童血清AMH浓度范围:外周性性早熟(PPP)女童为3.29(0.58,9.15)ng/ml,快速进展型-中枢性性早熟(P-CPP)女童为2.68(0.28,13.85)ng/ml,缓慢进展型-中枢性性早熟(SP-CPP)女童为4.07(1.31,8.94)ng/ml,缓慢进展型-青春期早发育(EP)女童为3.93(1.30,11.53)ng/ml,快速进展型-青春期早发育(EFP)女童为2.52(0.62,9.34)ng/ml,正常青春发育女童为3.51(0.78,11.75)ng/ml。Spearman相关性分析结果显示:在PPP、EP两个发育阶段,AMH浓度与卵巢容积(Vov)、子宫容积(Vut)及年龄均呈正相关(r=0.317,0.135,0.07,均P<0.05),与黄体生成素(LH)、卵泡刺激素(FSH)均呈负相关(r=-0.009,-0.213,均P<0.05);在CPP、EFP两个发育阶段,AMH浓度与Vov、LH、FSH、雌二醇(E_(2))、骨龄(BA)及年龄均呈负相关(r=-0.344,-0.448,-0.410,-0.156,-0.311,-0.228,均P<0.05)。结论 AMH在青春期前儿童异常发育阶段的水平总体随着年龄增长而上升,但下丘脑-垂体-性腺轴(HPGA)快速成熟可导致AMH浓度短暂降低。在中枢性性早熟阶段,AMH存在自我调节效应,使性发育向正常方向发展。
