低分子肝素治疗联合心理治疗对妊娠合并重型乙型肝炎患者的效果

The effect of low molecular weight heparin therapy combined with psychotherapy in pregnant patients with severe hepatitis B

目的分析低分子肝素治疗联合心理治疗对妊娠合并重型乙型肝炎患者的临床治疗效果及对患者心理状态的影响。方法招募2018年3月~2021年3月就诊本院的82例妊娠合并重型肝炎患者,分组依照随机数字表法(每组41例)。对照组接受常规治疗,观察组在对照组治疗基础上,接受低分子肝素+心理治疗,对比两组患者临床疗效、肝功能指标、焦虑自评量表(Self-Rating Anxiety Scale,SAS)评分、抑郁自评量表(Self-rating depression scale,SDS)评分、生活质量综合评定问卷(GQOLI-74)评分。结果观察组临床总有效率(95.12%)比对照组(73.17%)高,观察组治疗后肝功能指标(ALT、AST)均比对照组低(P<0.05);观察组治疗后SAS评分、SDS评分均比对照组低(P<0.05);观察组治疗后GQOLI-74评分比对照组高(P<0.05)。结论妊娠合并重型肝炎患者,在常规治疗基础上予以低分子肝素+心理治疗,可有效改善患者肝功能指标,减轻不良情绪,提高生活质量。

Objective To analyze the clinical treatment effect of low molecular weight heparin therapy combined with psychotherapy on patients with pregnancy combined with severe hepatitis B and their influence on the psychological status of patients.Methods 82 patients with pregnancy complicated with severe hepatitis who visited our hospital from March 2018 to March 2021 were recruited and divided into groups according to the random number table method(41 subjects in each group).The control group received routine treatment and the observation group received low molecular weight heparin+psychotherapy on the basis of the treatment of the control group.The clinical efficacy,liver function indexes,self rating Anxiety Scale(SAS),self rating Depression Scale(SDS)and comprehensive quality of life questionnaire(GQOLI-74)were compared between the two groups.Results The total clinical effective rate of the observation group(95.12%)was higher than that of the control group(73.17%),and the indexes of liver function(ALT and AST)in the observation group were lower than those in the control group(P<0.05).The SAS score and SDS score of the observation group were lower than those of the control group(P<0.05).After treatment,the GQOLI-74 score of the observation group was higher than that of the control group(all P<0.05).Conclusion The low molecular weight heparin combined with psychotherapy can effectively improve liver function indexes,reduce adverse emotions and improve the quality of life in pregnant women with severe hepatitis.
