摘要
目的探讨磁共振成像(MRI)参数与脑星形细胞瘤患者增殖细胞核抗原(PCNA)、基质金属蛋白酶-9(MMP-9)水平的相关性。方法采用回顾性研究方法,纳入2019年1月至2021年12月大同市第三人民医院收治的84例脑星形细胞瘤患者为观察组,选择同期入院疑似其他颅内肿瘤疾病体检者84名为对照组。统计两组入院时MRI参数[水肿指数(EI)、增强后相对信号强度(RSIGd)、信号强度增加百分率(EP)]、PCNA、MMP-9水平等。评价观察组病理分级,经世界卫生组织神经系统肿瘤分类标准分析,84例脑星形细胞瘤患者中,Ⅰ级19例,Ⅱ级20例,Ⅲ级35例,Ⅳ级10例,其中Ⅰ~Ⅱ级纳为低级组(n=39);Ⅲ~Ⅳ级纳为高级组(n=45)。对其不同病理分级程度患者MRI参数、PCNA、MMP-9水平进行对比,分析MRI参数与脑星形细胞瘤患者PCNA、MMP-9水平的相关性。结果观察组EI、RSIGd、EP值及PCNA、MMP-9水平为3.72±1.11、1.65±0.38、83.55±20.18、(48.11±10.76)%、(68.91±8.06)%,均高于对照组[1.04±0.23、0.84±0.21、21.36±8.74、(13.82±4.87)%、(32.16±5.44)%],差异均有统计学意义(P<0.05)。高级组EI、RSIGd、EP值及PCNA、MMP-9水平为3.81±1.23、1.72±0.42、85.91±21.76、(50.91±11.74)%、(73.33±7.91)%,均高于低级组[2.96±1.06、1.58±0.22、75.89±23.48、(39.72±12.08)%、(65.48±7.62)%],差异均有统计学意义(P<0.05)。经Pearson检验,EI、RSIGd、EP与PCNA、MMP-9呈正相关(r=0.268、0.453、0.815、0.174、0.539、0.621,P<0.05)。结论MRI参数与脑星形细胞瘤PCNA、MMP-9水平有一定相关性,可随着病理级别提高而增加。
Objective To investigate the correlation between magnetic resonance imaging(MRI)parameters and proliferating cell nuclear antigen(PCNA)and matrix metalloproteinase-9(MMP-9)levels in patients with astrocytoma.Methods Using a retrospective study,a total of 84 patients with cerebral astrocytoma who were admitted to the Third People's Hospital of Datong City from January 2019 to December 2021 were included as the observation group,a total of 84 patients with suspected other intracranlai tumor diseases who were admitted to the hospital during the same period for physical examination were selected as the control group.The levels of MRI parameters[edema index(EI),relative signal intensity after enhancement(RSIGd),percentage increase in signal intensity(EP)],PCNA,MMP-9 in the two groups at admission were counted.The pathological grades of the observation group were evaluated,according to the WHO classification criteria of nervous system tumors,among the 84 patients with astrocytoma,19 were grade I,20 were grade II,35 were grade III,and 10 were grade IV,among which gradesⅠ-Ⅱwere low-grade groups(n=39),gradesⅢ-Ⅳwere high-grade groups(n=45).And the levels of MRI parameters,PCNA and MMP-9 of patients with different pathological grades were compared,and the correlation between MRI parameters and PCNA and MMP-9 levels in patients with cerebral astrocytoma was analyzed.Results The values of EI,RSIGd,EP,PCNA,and MMP-9 in the observation group were 3.72±1.11,1.65±0.38,83.55±20.18,(48.11±10.76)%,(68.91±8.06)%,which were higher than those in the control group[1.04±0.23,0.84±0.21,21.36±8.74,(13.82±4.87)%,(32.16±5.44)%],the differences were statistically significant(P<0.05).The levels of EI,RSIGd,EP,PCNA and MMP-9 in the advanced group were 3.81±1.23,1.72±0.42,85.91±21.76,(50.91±11.74)%,and(73.33±7.91)%,which were higher than those in the low-level group[2.96±1.06,1.58±0.22,75.89±23.48,(39.72±12.08)%,(65.48±7.62)%],the differences were statistically significant(P<0.05).After Pearson test,EI,RSIGd,EP were positively correlated with PCNA and MMP-9(r=0.268,0.453,0.815,0.174,0.539,0.621,P<0.05).Conclusion MRI parameters have a certain correlation with the levels of PCNA and MMP-9 in cerebral astrocytoma,which can increase with the severity of the pathology.
