探讨DE-CCTA虚拟平扫评价老年冠状动脉钙化积分的应用准确性 被引量:1

摘要 目的 探讨双能量冠状动脉CT造影(DE-CCTA)虚拟平扫评价老年冠状动脉钙化积分的应用准确性。方法 选取我院2021年1月至12月收治的90例可疑性冠心病患者作为研究对象,全部患者在给予常规非对比增强CT钙化积分扫描之后再次进行回顾性的心电门控DE-CCTA扫描检查。采用Philips Intellispace Portal软件对DE-CCTA数据进行分析与处理,由此获得虚拟平扫的数据。指派两名工作经验在10年以上的影像科医师对常规非对比增强序列与虚拟平扫的钙化积分数值进行评估,对比右冠状动脉(RCA)、左主干(LAD)、回旋支(LCX)、前降支(ADA)在虚拟平扫序列以及非对比增强序列下的钙化积分与钙化体积,之后对虚拟评价与常规非对比增强序列钙化积分与钙化体积之间的相关性进行分析,同时对比两组患者的总体辐射剂量。结果 在RCA、LAD、LCX、ADA不同动脉下实施非对比增强序列与虚拟平扫相比钙化积分与钙化体积均较高,差异有统计学意义(P<0.05)。行Pearson相关性分析可见,虚拟平扫与常规非对比增强序列和所得到的钙化积分与钙化体积均具有密切的相关性(P<0.05)。虚拟平扫与常规非对比增强序列相比总体辐射剂量较少,差异有统计学意义(P<0.05)。结论 DE-CCTA虚拟平扫可用于对老年冠状动脉钙化积分量化评价,总体辐射剂量较少,但相比于常规非对比增强CT仍具有一定的劣势。 Objective To explore and analyze the application accuracy of dual-energy coronary computed tomography angiography(DE-CCTA)virtual plain scan in evaluating coronary artery calcium score in the elderly.Methods A total of 90 patients with suspected coronary heart disease who were admitted to our hospital from January 2021 to December 2021 were selected as the research subjects.All patients underwent prospective ECG gating again after conventional non-contrast-enhanced CT calcium score scanning.DE-CCTA scan check.The DE-CCTA data were analyzed and processed by Heart PBV software, thereby obtaining the data of virtual plain scan.Two radiologists with more than 5 years of work experience were assigned to evaluate the calcium scores of conventional non-contrast enhanced sequences and virtual plain scans, and compared the right coronary artery(RCA),left main trunk(LAD),circumflex branch(LCX),calcification score and calcification volume of anterior descending artery(ADA)under virtual non-contrast-enhanced sequence and non-contrast-enhanced sequence, and then the correlation between virtual evaluation and conventional non-contrast-enhanced sequence calcification score and calcification volume was analyzed.total radiation dose of patients in the group.Results Compared with virtual plain scan, the calcification score and calcification volume of non-contrast enhanced sequence under different arteries of RCA,LAD,LCX and ADA were higher, and the difference was statistically significant(P<0.05).Pearson correlation analysis showed that virtual plain scan and conventional non-contrast enhanced sequences were closely correlated with the obtained calcification score and calcification volume(P<0.05).Compared with conventional non-contrast-enhanced sequences, the overall radiation dose of virtual plain scan was less, and the difference was statistically significant(P<0.05).Conclusion DE-CCTA virtual plain scan can be used to quantitatively evaluate the calcium score of coronary artery in the elderly.The overall radiation dose is less, but it still has certain disadvantages compared with conventional non-contrast-enhanced CT.
