直肠癌Miles根治术后永久性肠造口患者自我感受负担及其影响因素

Self-perceived Burden and Its Influencing Factors in Patients with Permanent Enterostomy After Miles Radical Resection of Rectal Cancer

目的探讨直肠癌Miles根治术后永久性肠造口患者自我感受负担量表(SPBS)评分现况及其影响因素,并制定针对性干预策略,减轻其心理负担。方法前瞻性选取商丘市第一人民医院2021年5月至2022年2月117例行Miles根治术后永久性肠造口患者为研究对象,采用SPBS量表评估其自我感受负担,多元线性回归分析术后永久性肠造口患者SPBS评分影响因素。结果117例直肠癌Miles根治术后永久性肠造口患者SPBS评分为(37.52±1.47)分,处于中度水平。不同受教育程度、家庭亲密度评分、月收入水平、社会支持水平、生活质量、术前肠造口教育的永久性肠造口患者SPBS评分比较,差异有统计学意义(P<0.05)。受教育程度、生活质量、家庭亲密度评分、社会支持水平、月收入水平、术前肠造口教育是肠造口患者SPBS评分升高的影响因素(P<0.05)。结论直肠癌Miles术后永久性肠造口患者SPBS评分处于中等水平,主要与受教育程度、家庭亲密度评分、月收入水平、社会支持水平、生活质量、术前肠造口教育有关,建议临床据上述影响因素制定针对性护理措施,减轻心理压力,降低SPBS评分,促进其身心健康发展。

Objective To explore the self-perceived burden scale(SPBS)score and its influencing factors in patients with permanent enterostomy after Miles radical resection cancer of rectal cancer,and to develop targeted intervention strategies to reduce their psychological burden.Methods A total of 117 patients with permanent enterostomy after Miles from May 2021 to February 2022 were selected as the research objects.SPBS scale was used to assess their self-perception burden,and the influencing factors of SPBS score in patients with postoperative permanent enterostomy were analyzed by multiple linear regression.Results The SPBS score of 117 patients with Miles after radical resection of rectal cancer was(37.52±1.47)points,which was at a moderate level.SPBS scores of permanent enterostomy patients with different cultural level,family intimacy score,monthly income level,social support level,quality of life and preoperative permanent enterostomy education were compared,and the difference was statistically significant(P<0.05).Educational level,quality of life,family intimacy score,social support level,monthly income level and preoperative enterostomy education were the factors affecting the increased SPBS score in enterostomy patients(P<0.05).Conclusion SPBS score of patients with permanent enterostomy after Miles rectal cancer surgery is at a medium level,which is mainly related to education level,family intimacy score,monthly income level,social support level,quality of life and preoperative enterostomy education.It is suggested to develop targeted nursing measures according to the above influencing factors to reduce psychological pressure and reduce SPBS score.To promote their physical and mental health.
