单胎足月初产妇宫缩乏力性产后出血的高危因素

High Risk Factors of Postpartum Hemorrhage Caused by Uterine Asthenia in Singleton Full-Term Primiparas

目的探析单胎足月初产妇发生宫缩乏力性产后出血的高危因素。方法选取商丘市第四人民医院2018年3月至2022年3月收治的655例单胎足月初产妇作为研究对象,记录初产妇产后出血发生情况并分组,设计基线资料研究表,记录研究所需资料,采用logistic回归分析单胎足月初产妇宫缩乏力性产后出血发生的高危因素。结果655例单胎足月初产妇中有39例产妇发生宫缩乏力性产后出血,发生率为5.95%;发生组第三产程时间长于对照组,差异有统计学意义(P<0.05);发生组分娩前贫血、流产史、妊娠期高血压疾病以及与未发生组单胎足月初产妇比较,差异有统计学意义(P<0.05);建立logistic回归模型,结果显示,分娩前贫血、流产史、妊娠期高血压疾病以及第三产程时间≥15 min是单胎足月初产妇宫缩乏力性产后出血发生的高危因素(OR>1,P<0.05)。结论分娩前贫血、流产史、妊娠期高血压疾病以及第三产程时间≥15 min是单胎足月初产妇宫缩乏力性产后出血发生的高危因素。

Objective To explore the high risk factors of postpartum hemorrhage caused by uterine asthenia in singleton full-term primiparas.Methods A total of 655 singleton full-term primiparas treated in Shangqiu Fourth People’s Hospital from March 2018 to March 2022 were selected as the research subjects.The occurrence of postpartum hemorrhage of primiparas was recorded and divided into groups.The baseline data table was designed to record the data required for the study.Logistic regression was used to analyze the high-risk factors of postpartum hemorrhage caused by uterine asthenia in singleton full-term primipara.Results Thirty-nine of 655 singleton term primiparas had postpartum hemorrhage caused by uterine asthenia,with an incidence of 5.95%.The time of the third stage of labor in the occurrence group was longer than that in the control group,the difference was statistically significant(P<0.05).Compared anemia before delivery,abortion history,gestational hypertension and singleton term primiparas between the occurrence group and the non occurrence group,the difference was statistically significant(P<0.05).Logistic regression model was established,and the results showed that anemia before delivery,abortion history,gestational hypertension and the time of the third stage of labor≥15 minutes were the high-risk factors of postpartum hemorrhage caused by uterine asthenia in singleton term primiparas(OR>1,P<0.05).Conclusion Anemia before delivery,abortion history,gestational hypertension and the time of the third stage of labor≥15minutes are the high-risk factors of postpartum hemorrhage caused by uterine asthenia in singleton term primiparas.
