黎欣主任医师运用宁肝熄风汤治疗儿童抽动障碍经验

Experience of Chief Physician Li Xin in Treating Tic Disorder in Children with Ninggan Xifeng Decoction

目的总结黎欣主任医师运用宁肝熄风汤治疗儿童抽动障碍的临床经验。方法通过跟诊、收集病例,分析医案,从黎欣主任医师对抽动障碍病因病机的认识,以及运用宁肝熄风汤治疗儿童抽动症的方法,总结组方用药,探讨黎欣主任医师治疗此病的经验,并运用医案印证。结果黎欣主任医师认为儿童抽动症的发生与五脏相关,但主要病位在肝。外风可引动内风,主因肝郁、肝火、肝血虚致肝风内动而发病,故重在治肝。以自拟方宁肝熄风汤平肝息风,根据五脏症状及抽动部位进行加减,并辅以耳穴贴压及生活调护,以达到满意的治疗效果。所举医案患儿肝风内动,痰湿内蕴,治宜平肝熄风、祛风化痰,方选宁肝熄风汤加减,获得良好疗效。结论黎欣主任医师运用宁肝熄风汤为基础剂治疗儿童抽动障碍,临床疗效肯定,值得推广学习。

Objective To summarize the clinical experience of Chief Physician Li Xin in treating tic disorder in children by Ninggan Xiefeng decoction.Methods By following up,collecting cases and analyzing medical records,this paper summarized the prescription and medication of Chief Physician Li Xin from her understanding of the etiology and pathogenesis of tic disorder and the method of using Ninggan Xifeng decoction to treat children tic disorder,so as to explore the experience of Chief Physician Li Xin in treating this disease,and the medical records were used to verify it.Results Chief Physician Li Xin believed that the occurrence of tic disorder in children was related to the five viscera organs,but the main location of disease was liver.External wind can trigger the internal wind,but the main cause is endogenous liver wind which caused by liver depression,liver fire,liver and blood deficiency,so Li Xin focuses on the treatment of liver.The self-designed Ninggan Xifeng decoction was used to calm the liver and calm the wind.Addition and subtraction were performed according to the symptoms of the five viscera and the twitching sites.The treatment was supplemented with auricular point pressing and life conditioning to achieve a satisfactory treatment effect.The children in the case mentioned above with liver wind moved inside,phlegm and dampness accumulated.The treatment should be to calm the liver and extinguish the wind,dispel the wind and dissolve the phlegm.The prescription of Ninggan Xifeng decoction was added and reduced,and a good effect was obtained.Conclusion Chief Physician Li Xin used Ninggan Xiefeng decoction as the basic agent in the treatment of children with tic disorder.The clinical effect is positive and it is worth popularizing and learning.
