中医药干预DNP作用机制研究进展

Research Progress on the Mechanism of Traditional Chinese Medicine in the Intervention of Diabetic Peripheral Neuropathy

糖尿病周围神经病变(Diabetic peripheral neuropathy,DPN)是糖尿病最常见的慢性并发症之一,近年来在中国其发病率逐年增加。目前西医临床上尚无较为有效的治疗方法,多以对症治疗为主,中医历来强调整体观念、辨证论治且丰富的医家经验使中医在治疗DPN方面具有一定的优势,且现代生物学技术研究表明中医药多机制、多靶点的作用方式在治疗DPN的过程中起到重要作用。近年来,越来越多的中药复方及单味中药被证实可以通过调节免疫来防治DPN。此文整理了近年来中医药干预DPN作用机制研究成果,结果表明中医药可能通过调节线粒体能量代谢、减轻神经脱髓鞘病变程度、调节炎性因子分泌、抑制氧化应激来改善DPN相关症状。但相关的临床试验研究匮乏,仍有待进一步研究探索。

Diabetic peripheral neuropathy(DPN)is one of the most common chronic complications of diabetes and its incidence has been increasing year by year in China in recent years.The rich experience of traditional Chinese medicine(TCM)practitioners has given Chinese medicine an advantage in the treatment of DPN,and modern biological research has shown that the multi-mechanism and multi-target mode of action of Chinese medicine plays an important role in the treatment of DPN.In recent years,more and more TCM compounds and single herbs have been shown to prevent and treat DPN by modulating immunity.Therefore,this paper compiles recent research findings on the mechanisms of TCM intervention in DPN,which suggest that TCM may improve DPN-related symptoms by regulating mitochondrial energy metabolism,reducing the degree of neural demyelination,regulating inflammatory factor secretion and inhibiting oxidative stress.However,there is a lack of relevant clinical trial studies,and further research is still needed.
