清金化痰汤联合支气管镜肺泡灌洗术治疗重症肺炎患者的效果

Effect of Qingjin Huatan Decoction combined with bronchoscopic alveolar lavage on patients with severe pneumonia

目的探讨清金化痰汤联合支气管镜肺泡灌洗术治疗重症肺炎(SP)患者的临床效果。方法选取2020年8月至2021年10月新余市中医院收治的50例SP患者作为研究对象。采用随机数字表法将其分为观察组和对照组,每组各25例。对照组患者采用气管镜肺泡灌洗术治疗,治疗组患者采用气管镜肺泡灌洗术联合清金化痰汤治疗。比较两组患者的临床疗效以及治疗前及治疗后2周炎症因子水平、中医证候评分、肺部感染量表(CPIS)评分、急性生理学及慢性健康状况评分系统Ⅱ(APACHEⅡ)评分的变化情况。结果治疗组患者的治疗总有效率高于对照组,差异有统计学意义(P<0.05);治疗后治疗组的白细胞计数(WBC)、C反应蛋白(CRP)、降钙素原(PCT)水平均低于对照组,差异有统计学意义(P<0.05);治疗后治疗组的中医证候评分、CPIS评分、APACHEⅡ评分均低于对照组,差异有统计学意义(P<0.05)。结论清金化痰汤联合支气管镜肺泡灌洗术治疗SP疗效显著,值得临床推荐。

Objective To investigate the clinical effect of Qingjin Huatan Decoction Combined with bronchoscopic alveolar lavage in the treatment of patients with severe pneumonia(SP).Methods A total of 50 patients with SP admitted to Xinyu Hospital of Traditional Chinese Medicine from August 2020 to October 2021 were selected as the research objects.They were divided into observation group and control group according to random number table method,with 25 cases in each group.The patients in the control group were treated with bronchoscopic alveolar lavage,and the patients in the treatment group were treated with bronchoscopic alveolar lavage combined with Qingjin Huatan Decoction.The clinical efficacy and the changes of inflammatory factor level,traditional Chinese medicine syndrome score,pulmonary infection scale(CPIS)score,acute physiology and chronic health status scoring systemⅡ(APACHEⅡ)score before and 2 weeks after treatment were compared between the two groups.Results The total effective rate of the treatment group was higher than that of the control group,with statistically significant difference(P<0.05).After treatment,the levels of white blood cell count(WBC),C-reactive protein(CRP)and procalcitonin(PCT)in the treatment group were lower than those in the control group,with statistically significant differences(P<0.05).After treatment,the tradional Chinese medicine syndrome score,CPIS score and ApacheⅡscore of the treatment group were lower than those of the control group,and the differences were statistically significant(P<0.05).Conclusion Qingjin Huatan Decoction Combined with bronchoscopic alveolar lavage is effective in the treatment of SP,which is worthy of clinical recommendation.
