厄贝沙坦氢氯噻嗪联合美托洛尔在老年高血压引起重症心力衰竭患者中的应用效果

Application effect of Irbesartan Hydrochlorothiazide combined with Metoprolol in elderly patients with severe heart failure caused by hypertension

目的探讨厄贝沙坦氢氯噻嗪联合美托洛尔在老年高血压引起重症心力衰竭患者中的应用效果。方法选取2020年10月至2021年10月在吉林医药学院附属医院就诊的84例老年高血压引发重症心力衰竭患者作为研究对象,采用随机数字表法将其分为对照组(42例)和观察组(42例)。对照组患者采用常规治疗,观察组患者采用厄贝沙坦氢氯噻嗪联合美托洛尔治疗。比较两组患者治疗前后的脑利钠肽(BNP)、心功能指标水平;比较两组患者的治疗总有效率;比较两组患者的预后情况。结果观察组患者治疗后的BNP、左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)、左室射血分数(LVEF)低于对照组,差异有统计学意义(P<0.05);观察组患者的治疗总有效率(95.24%)高于对照组(71.43%),差异有统计学意义(P<0.05);观察组患者的全因死亡率、因心力衰竭再次入院率及脑卒中发生率均低于对照组,差异有统计学意义(P<0.05)。结论使用厄贝沙坦氢氯噻嗪联合美托洛尔治疗老年高血压引发重症心力衰竭患者,心功能指标有一定好转,治疗效果显著,预后良好。

Objective To investigate the application effect of Irbesartan Hydrochlorothiazide combined with Metoprolol in elderly patients with severe heart failure caused by hypertension.Methods A total of 84 elderly patients with severe heart failure caused by hypertension in the Affiliated Hospital of Jilin Medical University from October 2020 to October 2021 were selected as the research objects.They were divided into control group(42 cases)and observation group(42 cases)by random number table method.The control group received conventional treatment.Observation group was treated with Irbesartan Hydrochlorothiazide combined with Metoprolol.The levels of brain natriuretic peptide(BNP)and cardiac function indexes before and after treatment were compared between the two groups.The total effective rate of the two groups was compared.The prognosis of the two groups was compared.Results The BNP,left ventricular end-systolic diameter(LVESD)and left ventricular end diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF)in the observation group after treatment were lower than those in the control group,and the differences were statistically significant(P<0.05).The total effective rate of observation group(95.24%)was higher than that of control group(71.43%),and the difference was statistically significant(P<0.05).The all-cause mortality,readmission rate for heart failure and incidence of stroke in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Irbesartan Hydrochlorothiazide combined with Metoprolol in the treatment of elderly patients with severe heart failure caused by hypertension,the cardiac function indicators have improved to a certain extent,and the therapeutic effect is significant and the prognosis is good.
