体脂率对体外受精-胚胎移植临床妊娠率的影响研究

The effect of body fat percentage on clinical pregnancy rate of in vitro fertilization-embryo transfer

目的探讨体脂率(BFP)对不孕女性体外受精-胚胎移植(IVF-ET)临床妊娠率的影响。方法回顾性选取2016年5月至2017年12月于成都西囡妇科医院进行IVF-ET的1264例患者作为研究对象,根据BFP分为低BFP组(BFP<18%,n=63)、正常BFP组(BFP为18%~28%,n=534)和高BFP组(BFP>28%,n=667)。测量患者的BFP和体重指数(BMI),并收集其基线资料、辅助生殖相关指标和临床妊娠结局。采用多因素Logistic回归模型分析BFP对IVF-ET的影响;采用Pearson相关性分析探讨BFP与BMI的关系。结果1264例患者行IVF-ET临床妊娠率为46.4%(586/1264),正常BFP组临床妊娠率高于高BFP组(P<0.05)。多因素Logistic回归分析显示,BFP、BMI、年龄、不孕年限、人绒毛膜促性腺激素(HCG)日内膜厚度、移植胚胎数是IVF-ET临床妊娠率的影响因素(P<0.05)。Pearson相关性分析显示,BFP与BMI有显著正相关性(P<0.05)。结论高BFP可能会降低IVF-ET的成功率,建议助孕女性患者将BFP调整至正常范围;BFP与BMI密切相关,建议将两者结合使用。

Objective To evaluate the effet of body fat percentage(BFP)on the clinical pregnancy rate of in vitro fertilization-embryo transfer(IVF-ET)in infertile women.Methods A total of 1,264 patients who underwent IVF-ET in Chengdu Xinan Gynecological Hospital from May 2016 to December 2017 were retrospectively selected as the study objects.According to BFP,they were divided into low BFP group(BFP<18%,n=63)and normal BFP group(BFP 18%-28%,n=534)and high BFP group(BFP>28%,n=667).BFP and body mass index(BMI)were measured,and baseline data,indicators related to assisted reproduction and clinical pregnancy outcomes were collected.Multivariate Logistic regression model was used to analyze the effect of BFP on IVF-ET.Pearson correlation analysis was used to investigate the relationship between BFP and BMI.Results The clinical pregnancy rate of 1,264 patients undergoing IVF-ET was 46.4%(586/1,264),and the clinical pregnancy rate of the normal BFP group was higher than that of the high BFP group(P<0.05).Multivariate Logistic regression analysis showed that BFP,BMI,age,infertility years,human chorionic gonadotropin(HCG)daily intima thickness and the number of transplanted embryos were the factors affecting the clinical pregnancy rate of IVF-ET(P<0.05).Pearson correlation analysis showed that BFP was significantly positively correlated with BMI(P<0.05).Conclusions High BFP may reduce the success rate of IVF-ET.It is recommended that women treated with assisted reproductive technology should adjust the BFP to the normal range.BFP was significant correlated with BMI and a combination of the two is recommended.
