多层螺旋CT诊断急性下肢关节隐匿性骨折的应用价值分析

Application Value of Multi-Slice Spiral CT in the Diagnosis of Acute Occult Fracture of Lower Limb Joint

目的:分析多层螺旋CT诊断急性下肢关节隐匿性骨折的应用价值。方法:选取2020年12月~2021年12月本院收治的50例急性下肢关节创伤患者,所有患者均行DR平片诊断检查、多层螺旋CT诊断检查。比较不同诊断检查后的检查结果、阳性检出率。结果:多层螺旋CT诊断检查结果显示46例隐匿性骨折患者,4例非隐匿性骨折患者;46例隐匿性骨折患者中髋臼骨折、股骨头骨折、髂骨骨折、后踝骨折、跟骨骨折各有20例、14例、7例、3例、2例。DR平片诊断检查结果显示有27例隐匿性骨折患者,有23例非隐匿性骨折患者;27例隐匿性骨折患者中髋臼骨折、股骨头骨折、髂骨骨折、后踝骨折、跟骨骨折各有12例、8例、4例、2例、1例。多层螺旋CT诊断检查的阳性检出率高于DR平片诊断检查(P<0.05)。结论:急性下肢关节创伤患者应用多层螺旋CT检查能够提高隐匿性骨折的阳性检出率,可为后期临床治疗提供参考依据,诊断意义较高。

Objective:To analyze the application value of multi-slice spiral CT in the diagnosis of acute occult fractures of lower limb joints.Methods:50 patients with acute lower limb joint trauma treated in our hospital from December 2020 to December 2021 were selected.All patients underwent DR plain film diagnosis and multi-slice spiral CT diagnosis.The test results and positive detection rates after different diagnostic tests were compared.Results:The results of multi-slice spiral CT showed 46 patients with occult fracture and 4 patients with non occult fracture;Among 46 patients with concealed fracture,there were 20 cases of acetabular fracture,14 cases of femoral head fracture,iliac bone fracture,posterior malleolar fracture,and calcaneal fracture,respectively,7 cases,3 cases,and 2 cases.DR plain film showed that there were 27 patients with occult fracture and 23 patients with non occult fracture;Among the 27 patients with concealed fractures,there were 12 cases of acetabular fracture,8 cases of femoral head fracture,iliac fracture,posterior malleolar fracture and calcaneal fracture,4 cases,2 cases and 1 case respectively.The positive detection rate of multi-slice spiral CT was higher than that of DR plain film(P<0.05).Conclusion:The application of multi-slice spiral CT in patients with acute lower limb joint trauma can improve the positive detection rate of occult fracture,provide reference basis for later clinical treatment,and have high diagnostic significance.
