腹腔镜袖状胃手术治疗重度肥胖症的近期疗效分析

Analysis of Short-Term Curative Effect of Laparoscopic Sleeve Gastric Surgery for Severe Obesity

目的:分析重度肥胖症疾病特点,评价腹腔镜袖状胃手术(Laparoscopic Sleeve Gastrectomy,LSG)治疗的预后效果。方法:选择本院2018年2月~2019年3月进行手术治疗的80例重度肥胖症患者,经伦理委员会批准,进行随机法分组手术治疗。对照组患者采取腹腔镜Roux-en-Y胃旁路术(Laparoscopic Roux-en-Y Gastric Bypass,LRYGB)手术治疗,观察组患者采取LSG手术治疗。比较2组重度肥胖症患者的手术情况,包括不良反应情况以及手术前后不同时间点体质量参数情况、患者满意度情况。结果:组间不良反应情况比较,观察组患者吻合口漏、吻合口狭窄、内疝发生率均低于对照组(P<0.05),胃食管反流发生率高于对照组(P<0.05);组间术后不同时间点体质量、体质量指数情况较术前改善(P<0.05),组间术后差异并无显著性(P>0.05);组间术后患者问卷满意度问卷调查,观察组总满意度97.50%高于对照组的85.00%(P<0.05)。结论:LSG手术与LRYGB手术治疗重度肥胖症疾病症状改善效果基本相近,且安全性更高,整体疗效确切,满足患者治疗需求。

Objective:To analyze the characteristics of severe obesity disease and evaluate the prognostic effect of laparoscopic sleeve gastrectomy(LSG)treatment.Methods:80 patients with severe obesity who underwent surgical treatment in our hospital from February 2018 to March 2019 were selected and approved by the ethics committee for randomized surgical treatment.Patients in the control group were treated with laparoscopic Roux-en-Y gastric bypass(LRYGB)surgery,and patients in the observation group were treated with LSG surgery.The surgical conditions of the two groups of severely obese patients were compared,including adverse reactions,body mass parameters at different time points before and after the operation,and patient satisfaction.Results:The adverse reactions between the groups were compared.The incidence of anastomotic leakage,anastomotic stenosis,and internal hernia in the observation group were lower than those in the control group(P<0.05),and the incidence of gastroesophageal reflux was higher than in the control group(P<0.05);The body mass and BMI at different time points after the operation were improved between the groups before the operation(P<0.05),and there was no significant difference between the groups after the operation(P>0.05);The postoperative patient satisfaction questionnaire survey between the groups was observed.The total satisfaction of the group was 97.50%higher than that of the control group 85.00%(P<0.05).Conclusion:LSG surgery and LRYGB surgery have similar effects in the treatment of severe obesity disease symptoms,and have higher safety.The overall curative effect is definite,which can meet the treatment needs of patients.
