综合护理干预联合Buerger运动与中药熏蒸对竹叶青蛇咬伤患者下肢肿痛的影响

Effect of comprehensive nursing intervention combined with Buerger exercise andTraditional Chinese Medicine fumigation on lower limb swelling and pain of patients bitten by Trimeresurus stejnegeri bite

目的探讨综合护理干预联合Buerger运动与中药熏蒸对竹叶青蛇咬伤患者下肢肿痛的影响。方法选择2020年1月~2021年12月我院收治的竹叶青蛇咬伤下肢患者50例,随机分为对照组与观察组各25例。对照组在常规毒蛇咬伤救治基础上联合护理干预,观察组在对照组基础上给予患肢Buerger运动联合中药熏蒸,疗程均为14 d。观察比较两组患者入院后1、2周时的护理效果、疼痛视觉模拟评分(VAS)、心理状态(SAS、SDS)评分情况。结果护理干预2周后,观察组的总有效率明显高于对照组,差异有统计学意义(P<0.05)。护理干预后第1、2周时,观察组的VAS评分均明显低于对照组,差异均有统计学意义(均P<0.05)。护理后,两组患者的SAS、SDS评分均较护理前明显降低(均P<0.05),观察组的SAS、SDS评分降低更显著,与对照组比较差异均有统计学意义(均P<0.05)。结论综合护理干预联合Buerger运动与中药熏蒸对竹叶青蛇咬伤患者下肢肿痛的效果显著,能明显改善患者的临床症状,减轻疼痛程度,值得临床应用。

Objective To explore the effect of comprehensive nursing intervention combined with Buerger exercise and Traditional Chinese Medicine fumigation on lower limb swelling and pain of patients bitten by Trimeresurus stejnegeri bite.Methods From January 2020 to January 2022,50 cases of lower limb Trimeresurus stejnegeri bite in our hospital were randomly divided into the control group and the observation group with 25 cases each.The control group was given combined nursing intervention on the basis of conventional treatment of snakebite,and the observation group was given Buerger exercise combined with Chinese herbal fumigation on the basis of the control group.The course of treatment was 14 days.The nursing effect,visual analogue scale of pain(VAS)and psychological state(SAS,SDS)of the two groups were observed and compared one week and two weeks after admission.Results After two weeks of nursing intervention,the total effective rate of the observation group was significantly higher than that of the control group(P<0.05).One week and two weeks after the nursing intervention,the VAS score of the observation group was significantly lower than that of the control group(P<0.05 for both).The comparison of SAS and SDS scores between the observation group and the control group before and after nursing(P<0.05 for both),with significant difference.It is suggested that both nursing plans can improve the patients'unhealthy psychological state.Comparison between the two groups:After nursing intervention,the SAS and SDS scores in the observation group were significantly improved than those in the control group(P<0.05 for both).Conclusion Comprehensive nursing intervention combined with Buerger exercise and Traditional Chinese Medicine fumigation has a significant effect on lower limb swelling and pain of patients bitten by Trimeresurus stejnegeri bite.Which can significantly improve the clinical symptoms of patients,reduce the degree of pain,and reduce the pain of patients.It is worthy of clinical application.
