黄德弘教授治疗原发性头痛的遣方用药规律

Medication Rule of Professor Huang Dehong in the Treatment of Primary Headache

目的分析黄德弘教授运用中药治疗原发性头痛的特点,总结其用药规律。方法利用中医传承辅助平台管理系统(V2.5)软件,记录并分析黄德弘教授治疗头痛处方中的中药出现频次、性味、归经及证候、治则治法,归纳出方药使用频率。结果共纳入有效病例86例,112诊次。对112张处方中的144味中药进行频次统计,出现频次排名前5位为的川芎、当归、赤芍、蔓荆子、蒺藜。四气统计中,以温、平为主。五味统计中,以辛、甘为主。归经统计中,肝经、心经、脾经位居前3。药物组合统计中,“川芎-当归”“川芎-赤芍”“当归-赤芍”“川芎-蔓荆子”“蔓荆子-蒺藜”位居前5。结论黄德弘教授治疗原发性头痛时以“瘀、风”为病机,以活血、祛风为治则;注重肝心脾同调,从风、火(肝火、肝阳)、痰浊、血瘀、虚(脾虚、肾虚、气虚、血虚)5种发病机制入手,体现了中医辨证思想主抓病机,依法处方论治。

Objective To analyze the characteristics of Professor Huang Dehong in the treatment of primary headache,and and to summarize medication rules.Methods Using the management system software of traditional Chinese medicine inheritance assistance platform(V2.5),the occurrence frequency,taste,meridian return,syndrome and treatment method of traditional Chinese medicine in professor Huang Dehong’s prescription for treating headache were recorded and analyzed,and the frequency of prescription usage was summarized.Results A total of 86 effective cases were included with 112 diagnoses.The frequency statistics of 144 Chinese medicines in 112 prescriptions were carried out,and the top 5 were ligusticum wallichii,ligusticum,red peony root,fructus viticis and tribulus terrestris.In the four gas statistics,warm is the main ones.In the five taste statistics,spicy and sweet are the main ones.In the channel tropism statistics,the liver meridian,the heart meridian,the spleen meridian ranked the top three.“Ligusticum wallichii-ligusticum”,“ligusticum wallichii-red peony root”,“ligusticum-red peony root”,“ligusticum wallichii-fructus viticis”and“fructus viticis-tribulus terrestris”were the top 5.Conciusion When treating primary headache,Professor Huang Dehong took“blood stasis and wind”as the pathogenesis,and treated it with the academic idea of activating blood and eliminating wind.Focusing on the harmonization of liver,heart and spleen,starting with the five pathogenesis mechanisms of wind,fire(liver fire,liver yang),phlegm turbidity,blood stasis and deficiency(spleen deficiency,kidney deficiency,qi deficiency and blood deficiency),it reflects the main pathogenesis of traditional Chinese medicine syndrome differentiation and treatment according to law prescription.
