摘要
目的:对比分析MCS+血细胞分离机由少白细胞血小板采集程序(LDP)升级至通用血小板采集程序(UPP)前后血小板的采集效果,研究程序升级后采集血小板的质量及献血者安全。方法:通过采用自身对照的方式,选择分别采用LDP和UPP采集的80位献血者作为研究对象,比较两组程序采集血小板相关参数之间以及献血者发生不良反应率的差异,包括全血处理量、采后血小板计数、采集时间、ACD-A用量、1 h血小板采集量、采集效率以及献血不良反应发生情况等。结果:两组献血者在采集前全血容量、血细胞比容(HCT)、采前血小板计数方面比较,差异无统计学意义(P>0.05);UPP采后血小板计数与LDP采后血小板计数比较,差异无统计学意义(P>0.05)。UPP采集全血处理量、UPP采集时间、UPP ACD-A用量、UPP 1 h血小板采集量及UPP采集效率与LDP比较,差异有统计学意义(P<0.05);UPP采集时献血不良反应发生率与LDP比较,差异有统计学意义(P<0.05)。结论:UPP与LDP比较,通过升级采集模式,缩短了采集时间,提高了血小板收集效率,降低了献血不良的发生率。程序升级后各采血机构需要依据当地献血人群的具体情况及采集结果设置血细胞分离机采集相关参数,才能使升级后程序达到最佳采集效能,进一步提升献血者的舒适性和安全性。
Objective To compare and analyze the platelet collection effects of MCS + blood cell separator before and after upgrading from low leukocyte platelet collection program(LDP) to general platelet collection program(UPP),and to explore the quality of platelet collection and the safety of blood donors.Method Through self-control,80 blood donors who were collected by LDP and UPP programs were selected as the study subjects;The differences of platelet related parameters collected by the two programs and the adverse reaction rate of blood donors were compared,including the whole blood processing volume,platelet count after collection,collection time,ACD-A dosage,platelet collection volume in one hour,collection efficiency,and adverse reaction rate of blood donors.Results There was no significant difference in blood volume,hematocrit(HCT),pre harvest platelet count and post harvest platelet count between the two groups(P>0.05);There was no significant difference in platelet count between UPP and LDP(P>0.05).Compared with the LDP program,the whole blood treatment amount,collection time,dosage of ACP-A,platelet collection amount and collection efficiency of UPP in UPP collection were statistically significant(P<0.05);There was a statistically significant difference in the incidence of adverse reactions of blood donation between UPP program and LDP program(P<0.05).Conclusion Compared with the LDP program,the UPP program shortens the collection time,improves the platelet collection efficiency and reduces the incidence of poor blood donation by upgrading the collection mode;After the program is upgraded,each blood collection institution needs to set the relevant parameters of blood cell separator collection according to the specific situation of the local blood donors and the collection results,so that the upgraded program can achieve the best collection efficiency and further improve the comfort and safety of blood donors;The UPP program is worth promoting.
