高海拔地区经导管主动脉瓣置换术术后临床效果分析

Analysis on postoperative clinical effect of transcatheter aortic valve replacement in high altitude area

目的总结高海拔地区经导管主动脉瓣置换术患者的临床资料,对术后临床效果进行初步分析。方法回顾性分析2018年6月-2019年12月,云南省阜外心血管病医院行经导管主动脉瓣置换术患者的临床资料,评价术后并发症发生率,死亡率等临床效果。结果共13例患者(男9例,女4例),血管入路为11例经股动脉,2例经心尖。12例成功植入人工瓣膜,术中转开胸行冠脉移植术1例,植入瓣中瓣1例。术后并发症为肺部感染3例,下肢静脉血栓形成2例,Ⅲ°房室传导阻滞置入永久起搏器1例,死亡1例。结论高海拔地区患者行经导管主动脉瓣置换术治疗,临床效果显著。

Objective To summarize the clinical data of patients undergoing transcatheter aortic valve replacement(TAVR)in high altitude areas,and to preliminarily analyze the postoperative clinical effects.Methods The clinical data of patients who underwent TAVR at Fuwai Yunnan Cardiovascular Diseases Hospital from June 2018 to December 2019 were retrospectively analyzed.And the incidence of postoperative complications,mortality and other clinical effects were evaluated.Results A total of 13 patients(9 males and 4 females)were involved,the vascular access approach was via femoral artery in 11 cases and apex approach in 2 cases.12 patients were successfully implanted artificial valve,1 patient was converted to open thoracotomy for coronary artery transplantation,1 case of middle valve implantation.Postoperative complications included pulmonary infection in 3 cases,lower limb venous thrombosis in 2 cases,1patient with permanent pacemaker implantation inⅢ°atrioventricular block,1 patient died.Conclusions TAVR in high altitude area had significant clinical effect.
