349次住院患者不合理用药医嘱的干预和分析

Analysis on 349 interventions of irrational medication medical orders for inpatients

目的分析本院349次住院患者不合理用药医嘱的干预情况,提高临床合理用药水平。方法通过收集本院2020年6月-2022年6月期间药师对住院患者不合理用药医嘱的干预情况349次,并对住院患者不合理用药医嘱的干预应用Excel软件进行统计。结果医嘱干预回复率(98.28%),医嘱干预采纳率(85.96%);干预类型最多的是用法用量不适宜(83.38%);干预科室排名前三名分别是冠心病一病区(17.48%),心力衰竭病区(8.02%),肺血管与综合内科病区(7.16%)。结论药师在医嘱干预中要与同事、临床科室及医师加强沟通与交流,并聚焦住院患者不合理用药医嘱,针对性开展用药宣传,提高合理用药水平。

Objective To investigate 349 interventions of irrational medication medical orders for inpatients by pharmacists,so as to improve the level of clinical rational drug use.Methods The 349 interventions records of irrational medication medical orders for inpatients from June 2020 to June 2022 were collected,and statistical analysis was performed by Excel.Results The response rate of medical order intervention was 98.28%,the adoption rate of medical order intervention was 85.96%;The most intervention type of irrational drug use was inappropriate usage and dosage(83.38%);Top 3 intervention departments of irrational drug use were the first ward of coronary heart disease(17.48%),heart failure ward(8.02%),pulmonary vascular and general internal medicine ward(7.16%).Conclusions Pharmacists should strengthen the communication with colleagues,doctors and clinical department,and focus on inpatients with medical advice of irrational drug use,develop targeted medication publicity,improve the level of rational drug use.
