摘要
目的:分析胃癌(GC)患者围手术期血清外泌体miR-29b与根治性胃切除术后复发及组织基质金属蛋白酶-2(MMP-2)表达的相关性。方法:收集2011年1月至2017年5月于我院行根治性胃切除术的107例GC患者的临床数据进行分析。采用实时荧光定量聚合酶链反应及Western blotting分析血清外泌体miR-29b表达及组织MMP-2表达。随访观察GC患者术后复发情况。结果:GC组织MMP-2蛋白相对表达量高于癌旁组织[1.39(0.96,2.10)vs.0.77(0.56,1.38),P<0.001]。Spearman秩相关分析显示,GC患者血清外泌体miR-29b表达与GC组织MMP-2蛋白表达呈负相关关系(rs=-0.426,P<0.001)。单因素及多因素COX回归分析表明,血清外泌体miR-29b表达为GC患者根治术后复发的独立预测因子(P<0.001)。相对于血清外泌体miR-29b高表达组,低表达组患者术后的无复发生存期(RFS)更短(χ^(2)=29.177,P<0.001)。结论:血清外泌体miR-29b表达与组织MMP-2表达呈负相关关系,其可作为预测GC根治性胃切除术后复发的生物标志物。
Objective:To analyze the correlation between preoperative serum exosome miR-29b,recurrence after radical gastrectomy and tissue matrix metalloproteinase-2(MMP-2)expression in patients with gastric cancer(GC).Methods:Clinical data of 107 GC patients who underwent radical gastrectomy in our hospital from January 2011 to May 2017 were collected and analyzed.The serum exosomal miR-29b and tissue MMP-2 expression were analyzed by real-time quantitative polymerase chain reaction and western blotting.Postoperative recurrence of GC patients was observed during follow-up.Results:The expression of MMP-2 protein in GC tissues was higher than that in adjacent tissues[1.39(0.96,2.10)vs.0.77(0.56,1.38),P<0.001].Spearman rank correlation analysis showed that the expression of serum exosomal miR-29b in GC patients was negatively correlated with the expression of MMP-2 protein in GC tissues(rs=-0.426,P<0.001).Univariate and multivariate COX regression analysis showed that the expression of serum exosomal miR-29b was an independent predictor of recurrence after radical gastrectomy(P<0.001).Compared with the high expression group of serum exosome miR-29b,the low expression group had a shorter postoperative relapse-free survival(RFS)(χ^(2)=29.177,P<0.001).Conclusion:Serum exosomal miR-29b is negatively correlated with tissue MMP-2,and it can be used as a biomarker to predict recurrence after GC radical gastrectomy.
