摘要
目的:探究BAT评分在自发性脑出血(SICH)血肿扩大(HE)患者中的预测价值。方法:选取广州医科大学附属第六医院2017年4月—2021年3月收治的150例SICH患者,收集患者资料并记录B A T评分。将入组病例根据血肿有无扩大分为2组,采用单因素及多因素Logistic回归分析HE相关的危险因素,并采用受试者工作特征曲线(ROC)分析B A T评分对H E的预测价值。结果:入组病例中H E组55例,非H E组95例。单因素分析结果显示,HE组BAT评分高于非HE组,GCS分值低于非HE组;HE组使用抗凝药物比例高于非HE组,发病至首次CT时间短于非HE组,初始血肿体积大于非H E组,差异均有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,BA T评分是H E的独立危险因素(OR=1.520,95%C I:1.011~1.098,P<0.01)。BAT评分截断值为4分时,预测H E的敏感度为60.90%,特异性为76.40%,ROC曲线下面积(AUC)为0.746。结论:BAT评分是SICH早期HE的独立影响因素,并对HE有一定的预测价值。
Objective To explore the predictive value of BAT score for hematoma expansion(HE)in spontaneous intracerebral hemorrhage(SICH).Methods 150 patients with SICH were enrolled from April 2017 to March 2021,Baseline and clinical data were collected and BAT scores were recorded.The enrolled cases were divided into two groups according to whether the hematoma was enlarged or not.Single factor and multi factor logistic regression were used to analyze the risk factors related to HE,and the predictive value of BAT score on HE was analyzed using the subject’s work characteristic curve(ROC).Results Among the enrolled cases,55 cases were in HE group and 95 cases were in non HE group.Univariate analysis showed that in HE group,the BAT score was higher,the GCS score was lower,the proportion of anticoagulants used was higher,the time from onset to first CT was shorter,and the initial hematoma volume was larger.Multivariate logistic regression analysis showed that BAT score was an independent risk factor for hematoma enlargement(OR=1.520,95%CI:1.011~1.098,P<0.01).When the cut-off value of BAT score was 4 points,the sensitivity and specificity of predicting HE were 60.90%,76.40%,and the area under ROC curve(AUC)was 0.746.Conclusion BAT score is an independent influencing factor of early HE in SICH,and has certain predictive value for HE.
