光动力疗法治疗恶性皮肤肿瘤及癌前期皮肤病的临床效果及安全性

Clinical effect and safety of photodynamic therapy on malignant skin tumors and precancerous skin diseases

目的探究光动力疗法治疗恶性皮肤肿瘤及癌前期皮肤病的临床效果及安全性。方法选取2018年3月至2020年12月本院收治的64例恶性皮肤肿瘤及癌前期皮肤病患者作为研究对象,遵循随机数字表法将其分为参照组(n=32,手术治疗)和观察组(n=32,手术治疗+光动力疗法)。比较两组的治疗效果、不良反应发生情况、免疫功能指标、炎症因子水平及皮肤恢复状况满意度。结果观察组的治疗总有效率为96.88%,高于参照组的75.00%(P<0.05)。观察组的不良反应总发生率为3.12%,低于参照组的18.75%(P<0.05)。治疗后,两组的免疫球蛋白A(IgA)、免疫球蛋白M(IgM)、免疫球蛋白G(IgG)水平及白细胞分化抗原3阳性(CD3^(+))、白细胞分化抗原4阳性(CD4^(+))均升高,白细胞分化抗原8阳性(CD8^(+))均降低,且观察组优于参照组(P<0.05)。治疗后,两组的肿瘤坏死因子-α(TNF-α)、白细胞介素-4(IL-4)、白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)水平均降低,且观察组低于参照组(P<0.05)。观察组的皮肤恢复状况满意度为96.88%,高于参照组的81.25%(P<0.05)。结论在手术治疗基础上采用光动力疗法治疗恶性皮肤肿瘤及癌前期皮肤病的临床效果显著,可改善患者的免疫功能,降低血清炎症因子水平,提高患者满意度,且安全性较高。

Objective To explore the clinical effect and safety of photodynamic therapy on malignant skin tumors and precancerous skin diseases.Methods A total of 64 patients with malignant skin tumors and precancerous skin diseases admitted in our hospital from March 2018 to December 2020 were selected the research objects,and the patients were divided into reference group(n=32,surgical treatment)and observation group(n=32,surgical treatment+photodynamic therapy)according to the random number table method.The treatment effect,adverse reactions,immune function indexes,inflammatory factors levels and the satisfaction of skin recovery were compared between the two groups.Results The total effective rate of treatment in the observation group was 96.88%,which was higher than 75.00%in the reference group(P<0.05).The total incidence of adverse reactions in the observation group was 3.12%,which was lower than 18.75%in the reference group(P<0.05).After treatment,the levels of immunoglobulin A(IgA),immunoglobulin M(IgM),immunoglobulin G(IgG),cluster of differentiation 3 positive(CD3^(+))and cluster of differentiation 4 positive(CD4^(+))in the two groups increased,cluster of differentiation 8 positive(CD8^(+))decreased,and those in the observation group were better than the reference group(P<0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),interleukin-4(IL-4),interleukin-6(IL-6)and hypersensitivity C-reactive protein(hs-CRP)in the two groups decreased,and those in the observation group were lower than the reference group(P<0.05).The satisfaction of skin recovery in the observation group was 96.88%,which was higher than 81.25%in the reference group(P<0.05).Conclusion On the basis of surgical treatment,photodynamic therapy for malignant skin tumors and precancerous skin diseases has a significant clinical effect,it can improve the immune function of patients,reduce the levels of serum inflammatory factors,and improve patients'satisfaction,with high safety.
