血清抗CCP抗体、CCL19在类风湿性关节炎患者中的表达及意义

Expression and significance of serum anti-CCP antibody and CCL19 in patients with rheumatoid arthritis

目的探讨血清抗环瓜氨酸肽(CCP)抗体、趋化因子配体19(CCL19)在类风湿性关节炎(RA)患者中的表达及意义。方法选取2019年3月至2020年12月我院收治的54例RA患者作为RA组,另选取同期我院收治的53例其他关节炎患者作为Anti-RA组,以及50例健康体检者作为HC组。比较三组受检者的抗CCP抗体及CCL19水平,抗CCP抗体阳性率及抗CCP抗体阳性时的CCL19表达水平,抗CCP抗体、CCL19及抗CCP抗体联合CCL19对RA的诊断效能。结果RA组的抗CCP抗体、CCL19水平高于Anti-RA组和HC组,Anti-RA组的抗CCP抗体、CCL19水平高于HC组,差异具有统计学意义(P<0.05)。RA组的抗CCP抗体阳性率高于Anti-RA组和HC组,Anti-RA组的抗CCP抗体阳性率高于HC组,差异具有统计学意义(P<0.05);抗CCP抗体阳性时,RA组的CCL19水平高于Anti-RA组和HC组,Anti-RA组的CCL19水平高于HC组,差异具有统计学意义(P<0.05)。以临床诊断结果为金标准,抗CCP抗体联合CCL19对RA的诊断灵敏度、特异度及准确度高于抗CCP抗体和CCL19,差异具有统计学意义(P<0.05)。结论RA患者的血清抗CCP抗体、CCL19水平升高,抗CCP抗体阳性时,RA患者的CCL19水平升高,抗CCP抗体联合CCL19对RA的诊断效能较高,具有临床价值。

Objective To explore the expression and significance of serum anti-cyclic citrullinated peptide(CCP)antibody and C-C chemokine ligand 19(CCL19)in patients with rheumatoid arthritis(RA).Methods A total of 54 patients with RA admitted in our hospital from March 2019 to December 2020 were selected as RA group,during the same period,53 other arthritis patients admitted in our hospital were selected as Anti-RA group and 50 objects undergoing health screening as HC group.The levels of anti-CCP antibody and CCL19,the positive rate of anti-CCP antibody,the expression level of CCL19 in anti-CCP antibody positive,and the diagnostic efficacy of anti-CCP antibody,CCL19 and anti-CCP antibody combined with CCL19 in RA were compared among the three groups.Results The levels of anti-CCP antibody and CCL19 in the RA group were higher than those in the Anti-RA group and the HC group,the levels of anti-CCP antibody and CCL19 in the Anti-RA group were higher than those in the HC group,and the differences were statistically significant(P<0.05).The positive rate of anti-CCP antibody in the RA group was higher than that in the Anti-RA group and the HC group,the positive rate of anti-CCP antibody in the Anti-RA group was higher than that in the HC group,and the differences were statistically significant(P<0.05);when anti-CCP antibody was positive,the level of CCL19 in the RA group was higher than that in the Anti-RA group and the HC group,the level of CCL19 in the Anti-RA group was higher than that in the HC group,and the differences were statistically significant(P<0.05).Taking the clinical diagnosis results as the gold standard,the sensitivity,specificity and accuracy of anti-CCP antibody combined with CCL19 in the diagnosis of RA were higher than those of anti-CCP antibody and CCL19,and the differences were statistically significant(P<0.05).Conclusion The levels of serum anti-CCP antibody and CCL19 in RA patients increased,when anti-CCP antibody is positive,the level of CCL19 in RA patients increased,the diagnostic efficacy of anti-CCP antibody combined with CCL19 in RA is high,which has clinical value.
