脑梗死急性期患者采用半夏白术天麻汤加减联合西药治疗的临床效果及对患者生活质量的影响分析

Clinical Effect of Banxia Baizhu Tianma Decoction Combined with Western Medicine in the Treatment of Patients with Acute Cerebral Infarction and Its Influence on the Quality of Life of Patients

目的探讨半夏白术天麻汤加减联合西药治疗脑梗死急性期的临床效果,分析对患者生活质量的影响。方法选取2020年11月—2021年11月于北京中医药大学房山医院诊疗的108例脑梗死急性期患者为研究对象,通过随机综合平衡法分为两组,对照组(54例)患者应用常规西药进行治疗,研究组(54例)应用半夏白术天麻汤加减联合西药进行治疗。对比两组临床效果、治疗情况、生活质量、中医证候积分。结果研究组治疗总有效率94.44%明显高于对照组81.48%,差异有统计学意义(χ^(2)=4.258,P=0.038)。研究组治疗后的NIHSS评分明显低于对照组,日常生活能力(Barthel)、运动功能(Ful-Meyer)评分均明显较对照组高,差异有统计学意义(P<0.05)。研究组治疗后的各项脑卒中专用生活质量量表(SS-QOL)评分均明显高于对照组,差异有统计学意义(P<0.05);研究组治疗后的各项中医证候积分均明显低于对照组,差异有统计学意义(P<0.05)。结论予以脑梗死急性期患者半夏白术天麻汤加减联合西药治疗具有确切疗效,可有效改善中医证候积分,同时还可减轻神经损伤,促进运动功能、日常生活能力以及生活质量的改善,对患者预后改善具有重要意义。

Objective To investigate the clinical effect of modified Banxia Baizhu Tianma decoction combined with western medicine in the treatment of acute cerebral infarction,and analyze the impact on the quality of life of patients.Methods 108 patients with acute cerebral infarction diagnosed and treated in Fangshan Hospital of Beijing University of Traditional Chinese Medicine from November 2020 to November 2021 were selected as the research objects.They were divided into two groups by random comprehensive balance method.The control group(54 patients)was treated with conventional western medicine,and the study group(54 patients)was treated with modified Banxia Baizhu Tianma decoction combined with western medicine.The clinical effect,treatment,quality of life and TCM syndrome score of the two groups were compared.Results The total effective rate of 94.44%in the study group was significantly higher than that of 81.48% in the control group,and the difference was statistically significant(χ^(2)=4.258,P=0.038).The NIHSS score of the study group after treatment was significantly lower than that of the control group,and the scores of Barthel and Ful Meyer were significantly higher than that of the control group,and the difference was statistically significant(P<0.05).The scores of each stroke specific quality of life scale(SS-QOL)in the study group after treatment were significantly higher than those in the control group,and the difference was statistically significant(P<0.05);The scores of various TCM syndromes in the study group after treatment were significantly lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion It has a definite curative effect to treat patients with acute cerebral infarction by adding or subtracting Banxia Baizhu Tianma Decoction combined with western medicine,which can effectively improve the score of TCM syndromes,reduce nerve injury,promote the improvement of motor function,daily living ability and quality of life,and is of great significance to improve the prognosis of patients.
