小剂量阿奇霉素对慢阻肺患者肺功能及MUC5ac、TLR4蛋白表达水平的影响 被引量:1

Effects of Low-dose Azithromycin on Pulmonary Function and Expression Levels of MUC5ac and TLR4 in Patients with Chronic Obstructive Pulmonary Disease

摘要 目的探讨小剂量阿奇霉素对慢阻肺患者肺功能及MUC5ac、TLR4蛋白表达水平的影响。方法选取2020年2月—2021年2月广饶县中医院收治的108例慢阻肺患者为研究对象,采用随机数表法将其分为观察组和对照组,每组54例。给予对照组常规治疗方案,观察组在对照组基础上加用小剂量阿奇霉素。两组患者均连续治疗6个月。比较两组患者治疗后的临床疗效和血压、血脂水平,治疗前后的FEV1/FVC%、FEV1%、FEV1和TLR4、MUC5ac蛋白表达水平。结果治疗后观察组总有效率(92.59%)高于对照组(70.37%),差异有统计学意义(P<0.05);观察组患者的PAPs、PAPd、LDL-C、TG、TC水平低于对照组,HDL-C水平高于对照组,差异有统计学意义(P<0.05);治疗后两组FEV1/FVC%、FEV1%、FEV1水平相比治疗前呈升高趋势,且观察组高于对照组,差异有统计学意义(P<0.05);治疗后观察组患者的MUC5ac和TLR4蛋白表达水平分别为(2.13±0.13)、(1.94±0.23),均低于对照组的(3.89±0.22)、(2.75±0.66),差异有统计学意义(t=50.612、8.516,P<0.05)。结论小剂量阿奇霉素治疗慢阻肺患者可有效缓解临床症状,改善肺功能及肺部病变的吸收情况,抑制机体气道黏液的大量分泌,促进病情恢复。 Objective To investigate the effect of low-dose azithromycin on pulmonary function and expression levels of MUC5ac and TLR4 in patients with chronic obstructive pulmonary disease.Methods 108 patients with chronic obstructive pulmonary disease(COPD)admitted to Guangrao County Hospital of Traditional Chinese Medicine from February 2020 to February 2021 were selected as the research objects.They were randomly divided into observation group and control group with 54 patients in each group.The control group was given routine treatment,and the observation group was given low-dose azithromycin on the basis of the control group.Both groups were treated continuously for 6 months.The clinical efficacy,blood pressure,blood lipid level and the expression levels of FEV1/FVC%,FEV1%,FEV1 and TLR4,MUC5ac protein before and after treatment were compared between the two groups.Results After treatment,the total effective rate of the observation group(92.59%)was higher than that of the control group 70.37%,the difference was statistically significant(P<0.05);the levels of PAPs,PAPd,LDL-C,TG and TC in the observation group were lower than those in the control group,and the level of HDL-C was higher than that in the control group,the difference was statistically significant(P<0.05).After treatment,the levels of FEV1/FVC%,FEV1% and FEV1 in the two groups were higher than those before treatment,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).After treatment,the expression levels of MUC5ac and TLR4 protein in the observation group were(2.13±0.13)and(1.94±0.23)lower than those in the control group(3.89±0.22)and(2.75±0.66),respectively,the difference was statistically significant(t=50.612,8.516,P<0.05).Conclusion Low-dose azithromycin in the treatment of patients with COPD can effectively relieve clinical symptoms,improve pulmonary function and the absorption of pulmonary lesions,inhibit the massive secretion of airway mucus,and promote the recovery of the disease.
