医院病房药房药学服务深化探索

Deepening Exploration of Pharmaceutical Care in a Ward Pharmacy of a Hospital

目的 为探索药学服务模式及深化药学服务转型发展提供参考。方法 医院病房药房与信息中心合作,在医院信息系统内开发出院患者用药指导核心信息数据库;建立专用物流配送信息系统,提供腹膜透析患者居家用药配送服务;引进院内药品配送闭环管理系统,优化药品配送流程。结果 患者在出院前能获得所带药品的完整用药信息,出院后实现了用药的自我监护及在线远程咨询,降低了用药安全风险;有效解决了腹膜透析患者携带腹膜透析液不便的问题;减轻了药房药师和病区护士的工作量,提高了药品配送的时效性,实现了药品院内流通的全程可追溯。结论 该院药学部病房药房改进模式后,药学服务工作获得提质增效。该研究为医院药学服务的创新及转型发展提供了参考。

Objective To provide a reference for exploring pharmaceutical care mode and deepening the transformation and development of pharmaceutical care.Methods The hospital ward pharmacy cooperated with the information center to develop the core information database of medication guidance for discharged patients in the hospital information system(HIS),establish a dedicated logistics distribution information system for providing medication delivery service for peritoneal dialysis patients at home,and introduce the closed-loop management system of hospital drug delivery for optimizing the drug delivery flow.Results Patients could be provided complete medication information before discharge,the self-monitoring of medication and online remote consultation service could be realized after discharge,and the medication risk could be reduced. The inconvenient problem of carrying peritoneal dialysate in patients with peritoneal dialysis was effectively solved. The workload of pharmacists and ward nurses was alleviated,the timeliness of drug delivery was improved,and the full traceability of drug circulation in the hospital was realized.Conclusion After the improvement of the model,the quality and efficiency of pharmaceutical care in the ward pharmacy of the pharmacy department of the hospital have been improved. This study provides a reference for the innovation,transformation and development of pharmaceutical care.
