24248例体检妇女HPV感染现状及宫颈细胞学结果分析

Analysis of the human papilloma virus infection status and the result of cervical cytology of 24248 women underwent physical examination

目的:探讨本地区健康人群宫颈人乳头瘤病毒(HPV)感染现状及宫颈细胞学异常情况。方法:对2016-2021年本院健康体检的24248例女性HPV感染情况及宫颈液基细胞学检测(TCT)异常情况进行回顾性分析。结果:HPV总感染率为13.4%。2016年感染率最高(18.0%),2019年感染率最低(10.1%),不同年份HPV感染率比较,差异有统计学意义(χ^(2)=127.46,P<0.000)。≤25岁人群感染率最高(29.0%),≥66岁人群感染率次之(15.2%),不同年龄组感染率差异有统计学意义(χ^(2)=38.14,P<0.000)。HPV高危型和低危型感染率分别为14.5%和3.3%。HPV高危型感染率前5位分别为16型(3.8%)、52型(2.6%)、53型(1.5%)、58型(1.5%)、18型(1.4%)。TCT异常检出率为2.4%。ASC和HSIL检出率较高的年龄段为46~55岁,LSIL检出率最高为≤25岁组。结论:本院健康女性中HPV高危型以16、52、53、58、18型为主,46~55岁人群中HSIL检出率最高,提示应重视本地区HPV高发感染型别和HSIL高发人群的宫颈癌筛查。

Objective:To survey the situations of human papilloma virus(HPV)infection and the abnormal cervical cytology of health women.Methods:A retrospective analysis was conducted on the situations of HPV infection and abnormalities of cervical TCT of 24,248 healthy women who underwent physical examination in our hospital during 2016-2021.Results:The overall rate of HPV infection of the women was 13.4%.The infection rate of the women in 2016 was the highest of 18.0%,and the infection rate of the women in 2019 was the lowest of 10.1%.The HPV infection rate of the women in different years was statistically significant difference(χ^(2)=127.46,P<0.000).The highest HPV infection rate of 29.0% was found in the women≤25 years old,followed by that(15.2%)of the women≥66 years old.There was significant difference in the HPV infection rate among the women with different age(χ^(2)=38.14,P<0.000).The infection rates for high-risk and low-risk types of HPV of the women were 14.5% and 3.3%,respectively.The top 5 infection rates of high-risk types HPV were 16(3.8%),52(2.6%),53(1.5%),58(1.5%),and 18(1.4%),respectively.The abnormal detection rate of TCT of the women was 2.4%.The age of the women with high detection rate of ASC and HSIL was 46-55 years old,and the highest detection rate of LSIL of the women was in≤25 years old.Conclusion:Among the healthy women who receive the regular health check,the high-risk HPV types infection are mainly 16,52,53,58,and 18.The HSIL has the highest detection rate among the women between 46 to 55 years old,indicating that the local region should pay attention to the cervical cancer screening for the women with specific HPV infections types and for the women with HSIL.
