磁共振成像与CT扫描在急性颅脑损伤诊断中的临床价值比较 被引量:4

摘要 目的分析急性颅脑损伤患者诊断中采用磁共振成像(MRI)与CT扫描的价值。方法100例急性颅脑损伤患者,均实施MRI、CT扫描检查。以手术病理结果为金标准,比较两种方法对急性颅脑损伤的阳性检出率、诊断效能及对损伤部位(颞叶、额叶、顶叶、枕叶)、血肿厚度(<1.5 mm、1.5~5.0 mm、5.1~8.0 mm、>8.0 mm)的诊断准确率。结果MRI对急性颅脑损伤的阳性检出率为96.00%,高于CT扫描的82.00%,差异具有统计学意义(P<0.05)。MRI对急性颅脑损伤的诊断准确率96.00%高于对照组的74.00%,漏诊率4.00%、误诊率0低于对照组的18.00%、8.00%,差异具有统计学意义(P<0.05)。MRI对损伤部位颞叶、额叶、顶叶、枕叶的诊断准确率分别为94.34%、96.00%、100.00%、91.67%,均高于CT扫描的81.13%、76.00%、60.00%、50.00%,差异具有统计学意义(P<0.05)。MRI对血肿厚度<1.5 mm、1.5~5.0 mm、5.1~8.0 mm、>8.0 mm的诊断准确率分别为86.67%、90.00%、94.00%、100.00%,均高于CT扫描的76.67%、60.00%、76.00%、73.33%,差异具有统计学意义(P<0.05)。结论相较于CT扫描检查,MRI在急性颅脑损伤诊断中更具优势,不仅可提高诊断准确率,亦可准确评估其血肿厚度,为临床医师诊断及治疗提供参考,可作为有效诊断方法应用于临床。 Objective To analyze the value of magnetic resonance imaging(MRI)and CT scan in the diagnosis of acute head injury.Methods MRI and CT scan were performed in 100 patients with acute head injury.Taking the surgical and pathological results as the gold standard,the positive detection rate,diagnostic efficacy and diagnostic accuracy for injury site(temporal lobe,frontal lobe,parietal lobe,and occipital lobe),hematoma thickness(<1.5 mm,1.5-5.0 mm,5.1-8.0 mm,>8.0 mm)of acute head injury of the two methods were compared.Results The positive detection rate of MRI for acute head injury was 96.00%,which was higher than 82.00%of CT scan,and the difference was statistically significant(P<0.05).The diagnostic accuracy of MRI for acute head injury was 96.00%,which was higher than 74.00%of the control group;the missed diagnosis rate of MRI for acute head injury was 4.00%,and the misdiagnosis rate was 0,which were lower than 18.00%and 8.00%of the control group;the differences were all statistically significant(P<0.05).The diagnostic accuracy of MRI for the injury site of temporal lobe,frontal lobe,parietal lobe,and occipital lobe of were 94.34%,96.00%,100.00%,and 91.67%,which were higher than 81.13%,76.00%,60.00%,and 50.00%of CT scans,and the differences were all statistically significant(P<0.05).The diagnostic accuracy rates of MRI for hematoma thickness<1.5 mm,1.5-5.0 mm,5.1-8.0 mm,and>8.0 mm were 86.67%,90.00%,94.00%,and 100.00%,which were higher than 76.67%,60.00%,76.00%,and 73.33%of CT scan,and the differences were all statistically significant(P<0.05).Conclusion Compared with CT scan,MRI has more advantages in the diagnosis of acute head injury.It can not only improve the diagnostic accuracy,but also accurately assess the thickness of the hematoma,providing a reference for the diagnosis and treatment of clinicians,and can be used as an effective diagnostic method in clinical practice.
